1. We have recorded ambulant intra-arterial blood pressure in five normo- and 14 hyper-tensive subjects over 24 h, particular attention being paid to the time of arousal. 2. The data were edited beat-by-beat before storing systolic, diastolic, and mean blood pressure +/- SD for every 2 min period on a computer file. 3. When pooled hourly mean data are plotted there is a rise of 52.9 mmHg systolic and 22.3 mmHg diastolic pressure from 03.00 hours to 11.00 hours in the 14 hypertensive subjects. 4. When the data are plotted in 20 min periods for the hours before and after arousal it is clear that the major rise in pressure occurs abruptly at that time. The 2 h period before arousal shows only a small rise in pressure, i.e. 6.9 mmHg systolic and 2.9 mmHg diastolic. 5. It is suggested that the apparent rapid early morning rise of blood pressure is an artifact of the method of analysis.

To study the applicability of simple field measurements of physical fitness in a sample of Omani boys and their relationships to selected variables. Two field measures of physical fitness-the time to complete 1.6-km run/walk and the sum of 5 skinfold thicknesses-were correlated with personal and family physical activity-related and other variables in a sample of 109 Omani boys aged 9-11 years. Obesity in parents, especially in the mother, showed significant correlation with both fitness measures. The waist/buttocks ratio showed no significant correlation with the 1.6-km time. The number of siblings and siblings sharing a room, number of T.V-watching hours and the number of servants in the family showed no correlation with the chosen fitness indices. The results of this pilot study indicate that simple field fitness tests can be used in children and they can yield valuable information related to physical fitness. Eleven normotensive diabetics with noninsulin-dependent diabetes mellitus (NIDDM) (mean age 52.5 +/- SD 8.2 years) and 11 controls (mean age 47.4 +/- SD 8.9 years) had their ambulatory blood pressure and heart rate recorded non-invasively by the Oxford Medilog System in standard hospital conditions. The results were averaged as hourly means of systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial blood pressure (MAP), and heart rate (HR) for the 24-h period and similarly for the 'awake' period (14-16 h) and the 'asleep' period (8-10 h). Hourly means for diabetics and controls showed no differences in blood pressure and heart rate over the 24 h. During sleep, control subjects showed a significant drop in SBP (P < 0.001), DBP (P < 0.001), MAP (P < 0.001) and HR (P < 0.001). To describe the angiotensin converting enzyme (ACE) genotype frequencies among Omani Arabs. A polymerase chain reaction (PCR) test, based on separation of different size DNA fragments, was developed to test the presence or absence (polymorphism) of a small DNA deletion in the ACE gene. The subjects were 124 Omani Arab students of Sultan Qaboos University, Muscat. The frequency of the I allele was 0.29, while that of the D allele was 0.71. The gene frequency distribution did not deviate from Hardy-Weinberg equilibrium. The frequency of D allele among Omanis is similar to that among other Arabs and Africans, but differs significantly from that among the Japanese and Chinese.

Mortality from cardiovascular disease in the Middle East (ME) is projected to increase substantially by 2020. There are no large studies on the impact of risk factors for acute myocardial infarction (AMI) in the region. This is a report on the association of nine risk factors with AMI in the ME. As part of the INTERHEART (IH) study, we enrolled 1364 cases of first AMI and 1525 matching controls from eight ME countries. The age at first AMI was 51.2 ± 10.3 years, which is the youngest, and with the largest proportion of patients <40 years in the entire IH population. The overall population attributable risk (PAR) of the nine risk factors to AMI was higher in the ME (97.5%) than worldwide (90.4%). Elevated apolipoprotein (Apo)B/ApoA1 had the strongest association with AMI, with odds ratio (OR) of 3.43 and PAR of 57.1%, followed by smoking (OR 3.63 and PAR 45.6%).
