ACC Nursing Home Receives State’s Prestigious Quality Award By Melanie Henderson

In May, the California Department of Health Services awarded ACC Nursing Home a special Quality and Accountability Supplemental Payment (QASP) for the period of July 2012 through June 2013. Out of the approximately 1,100 California skilled nursing facilities that were potentially eligible, only 114 facilities qualified for the award based on meeting the highest quality standards set by the State. The intent of the QASP program is to provide tangible recognition of, and incentives for, nursing homes that provide a higher quality of care as evidenced by meeting specific quality criteria. There is recognition that

regulatory enforcement efforts alone do not ensure quality of care in California’s skilled nursing facilities, but recognizing and rewarding the best facilities works to promote quality. ACC is proud of the care and concern the staff at the Nursing Home provide to our residents on a daily basis. Person-centered care and respect for elders is the key to all the decisions we make and policies we implement. We couple this personcentered care with a total commitment to meeting or exceeding the highest quality standards. We are proud to be recognized as one of the best nursing homes in California.

Legacy Corps team is full of personality!

Legacy Corps members attended a retreat to define and reflect on the meaning of teamwork. Since November 1st, the group has donated approximately 3,000 hours of service providing respite to caregivers and educating the community about Legacy Corps. We are proud of their accomplishments and appreciate

their dedication in supporting caregivers and seniors. With their help, many family caregivers are able to take a break, knowing that their loved ones are safe with volunteers and are engaged in meaningful activities and companionship. (continued on page 5)

In the last ACC Newsletter, we shared with you the opening of the Assisted Living at ACC Greenhaven Terrace in May of this year. Our Assisted Living is comprised of 24 units with onebedroom units in six different floor plans to choose as well an activities area and a dining area. Assisted Living is fully staffed with wonderful and caring staff who assist the residents with their day-today needs as well as coordinating wellness, educational, and recreation activities. We are signing up new residents. Make an appointment and visit Assisted Living at ACC Greenhaven Terrace or attend one of a series of open houses that ACC Senior Services is having for the community to visit, see the living quarters, and ask questions. I am very proud of ACC Senior Services as ACC continues to meet the needs of older adults and the community. I would like to take this time to thank Alan Keang and Stan & Florence Quon for their generous donations to the ACC Nursing Home. ACC has recently recognized them as Medallion Donors to the Nursing Home. ACC Senior Services values the support and donations we receive to continue to provide services to the older adult community. This is very much appreciated by the ACC Senior Services Board of Directors. As we close out the summer, I want to share with you that ACC Senior Services held a Strategic

President’s Message

Retreat on June 21, 2014, that was attended by many people from various areas of ACC Senior Services: Board Members, Chief Executive Officer, Chief Operating Officer, Controller, Staff Members, Committee Chairs, and some Committee Members. We received an update on the services ACC provides, and discussed challenges on the horizon and opportunities that ACC can strategically leverage. ACC Senior Services offers an array of programs and services to help older adults, and the strategic retreat allowed us to discuss what ACC will look like in the future. We will be establishing study groups to continue the strategic discussion and develop plans for next steps. We will continue to provide updates to you. To start out the fall season, ACC has the Annual Nursing Home Fundraiser Dinner on September 13, 2014, held at the Sacramento Asian Sports Foundation in Elk Grove. The Nursing Home Fundraiser Committee has been busy meeting and

planning another wonderful evening for the dinner. For the first time this year we will have a raffle and as well as silent auction items. Be sure to check out the raffle items and purchase raffle tickets to win that special item that you just can’t be without. ACC Senior Services always has an event planned, and after the Nursing Home Fundraiser Dinner, the ACC Craft & Bake Sale is right around the corner on November 7, 2014 from 9 a.m. to 4 p.m. Stop by and pick out a craft item or plant, buy a raffle ticket, pick out a baked goodie, or have lunch at the Craft and Bake Sale. This is a fun event. See you there. As summer comes to an end and we head into the fall season, take a few moments to reflect back on accomplishments, vacations taken, a get together or two with friends and family, attending or volunteering at one of the many bazaars in our community, or volunteering in our community. Take a moment to catch your breath and give thanks for the wonderful memories you have. I will, as this has been a busy summer for me. In closing, fall is a special time of year as the hustle and bustle of summer ends and we see the leaves start to turn colors. We notice the evenings start to get cooler and we have to get that light sweater out to keep the chill out. So, eat well, exercise, and stay healthy!

Jean Shiomoto Confirmed DMV Director Jean has worked in state government for 34 years, 26 of which have been in increasingly responsible positions at the DMV. Senate Rules Committee member Senator Steve Knight attributed the lower number of complaints about the DMV in his office to Jean’s leadership at the DMV. Even as she oversees a department with over 900 employees statewide, she makes time to serve as the President of the Board at ACC Senior Services. “We are fortunate to have her leadership and benefit from her talent, whether it is development of the ACC website or establishing a social media presence, or heightening our Jean Shiomoto (left) and Donna Yee (right) attention to person-centered care of elders and their family members, or assuring accountability for care provided and Jean Shiomoto, President of the ACC Senior resources needed,” stated Donna L. Megan Lucas joined ACC Greenhaven Terrace (GT) as Community Manager on July 28, 2014. For the past 3.5 years, Megan worked in the senior service industry at Revere Court, formerly known as Primrose. She is excited to be a part of GT and looks forward to helping us continue making GT a great place to live for seniors. Megan was born and raised in Sacramento and is married to Brandon who she says is the nicest man she knows! They will be celebrating their two-year anniversary on September 15th. Megan loves to visit with family and friends, travel, and eat. Working in the senior industry brings her great joy, and she is excited to get to know our residents and extended ACC family!

You Can Make a Difference Through Workplace Giving

Workplace giving is one of the easiest ways to give to charity, and some companies will even match their employees’ donations. Every dollar is important, and employer matching gifts are a wonderful way for individuals to increase the benefit their contributions provide for their charity of choice. ACC Nursing Home Honors Newest Medallion Donors: Stan and Florence Quon

Stan and Florence Quon

On July 29, 2014, ACC held a celebration to honor Stan and Florence Quon who are the newest donors to be recognized on the ACC Nursing Home’s Medallion Donor plaque. The Medallion Donor plaque acknowledges donors who have given a gift of $25,000 or more to the ACC Nursing Home.

Stan and Florence have been Jade sponsors of the ACC Nursing Home Fundraiser for many years and have heard about the nursing home’s excellent reputation. However, they never had firsthand experience until recently. Florence had a hip replacement surgery and because of her age, 90 next year, her husband Stan was concerned whether she could tolerate the stress of the surgery. In addition, Florence had gone through some bad experiences with other nursing homes following prior

surgeries. After some research, they decided ACC Nursing Home would be the most ideal nursing facility for Florence to recover from her hip replacement surgery. “The ACC Nursing Home facility, services, care and compassion were beyond our expectation. Florence believes the care at ACC helped significantly expedite her recovery. As a token of our appreciation, we have made a financial donation to enable ACC to continue to provide the highest quality care to support older adults. We hope our contribution will inspire others in the community to support ACC’s worthwhile endeavors” said Stan Quon. Mr. and Mrs. Quon located themselves in Sacramento from San Francisco in the early 1970s and were married in 1974. Stan served as a State Administrator for 30 years and as an adjunct professor at Sacramento City College for 40 years. Florence served as a tax auditor for The Employment Development Department for 30 years. They settled in the Lake Greenhaven Community and have been there ever since. ACC wishes to thank Stan and Florence Quon for their very generous gift of support to the ACC Nursing Home.

Dedicated Volunteer Leaves Bequest To ACC

ACC Senior Services held a small reception on May 29 with friends and family of Mr. Alan Keang to celebrate a very generous bequest from Mr. Keang’s estate to the Nursing Home. ACC recognized Mr. Keang’s generosity at the reception by honoring the addition of his name to the Medallion Donor Wall at the Nursing Home, which signifies a gift of $25,000 or more. Mr. Keang was dedicated to ACC. He volunteered many years as a Bingo caller and was a member of the ACC Volunteer Hall of Fame. When his health issues required skilled nursing care, the family said there was no doubt that ACC Nursing Home was his facility of choice. According to Mr. Keang’s brother Rudy, Alan had planned to leave a significant portion of his estate to ACC prior to becoming ill because of the respect he had for ACC and for the volunteers with Alan Keang (left) whom he worked. “We are so grateful for Mr. Keang’s generous support that will benefit so many others for years to come,” remarked Donna Yee, CEO of ACC. “It is especially meaningful that Mr. Keang was involved in the early days of raising money for the Nursing Home and has now continued that commitment through his estate plan.” No matter the size of your estate, a plan can help you ensure that your assets go to the people and organizations you care about while minimizing the amount that goes to the government. Are There Fraudulent Charges on Your Phone Bills? By Susan Sarinas, Program Manager

According to an article that appeared in the Sacramento Bee on July 2, 2014, Anne Flaherty, of The Associated Press, wrote that T-Mobile was “the target of a federal investigation and lawsuit” and it was alleged that they “bilked potentially hundreds of millions of dollars from its customers in fraudulent charges.” The article went on to say that T-Mobile allegedly “billed consumers for subscriptions to premium text services such as $10-per month horoscopes or updates on celebrity gossip that were never authorized by the account holder.” The practice of putting unauthorized charges on a person’s residential or mobile telephone is also known as “cramming.” If you think this is a rare occurrence, you are sadly mistaken. Just last year, AT&T was involved in a Third Party Billing Settlement which affected all AT&T landline telephone customers who were billed for ThirdParty Charges between January 1, 2005, and January 14, 2013. Every month we see at least one or two clients with charges on their bills that they do not recognize. We have seen charges for horoscopes, music downloads, and email discount coupons. Just last month, a client brought her cell phone bill for me to review. She was paying for her phone and the phones of her two granddaughters. I discovered a monthly charge of $9.99 on one granddaughter’s phone for a Travel Pack, which included Roadside Assistance and Navigator. Unfortunately, that

granddaughter was legally blind, unable to drive, and had absolutely no need for the Travel Pack on her phone. If I hadn’t asked the client about the charge, she would have just kept paying it every month. She assumed the bill was correct. Over the course of a year, this client would have paid over $119 for a service that her granddaughter was not using! What can you do to protect yourself from unauthorized “Third Party Charges?” The most important thing you can do is to check your phone bills every month for any charges that are out of the ordinary. If you are not sure what charges should or should not be on your bill, you can make an appointment to have your phone bill reviewed here at ACC Senior Services Center. We can check your bills and make sure there are no unusual charges. We may also be able to tell you how you can save money on your phone bills. There is no charge for this service. The Telecommunications Education and Assistance in Multiple-Languages (TEAM) program provides free education and assistance with telephone services to seniors and anyone with limited English proficiency. The program is funded by the California Public Utilities Commission. OTOW ORCHARD Located in Granite Bay, Otow Orchard is family-owned and operated. They sell peaches, plums, persimmons, and other tree fruit. They specialize in the ancient art of dried persimmons known as hoshigaki. Their fresh persimmons are dried each fall in a slow, patient, hands-on process that usually takes three to six weeks...per persimmon. Charitable Gifting: Overview and Tax Implications Overview The desire to assist a charitable organization must be a primary motive for making a gift; if a charitable inclination does not exist, charitable giving is difficult to justify. On the other hand, if a charitable motive does exist, the tax laws provide various methods and incentives to encourage the donor’s goodwill. The following summarizes the general rules applicable to charitable giving, including a discussion of some of the more common strategies used. Tax Implications- Charitable Deduction Rules Generally, charitable gifts can reduce federal income, gift, and/or estate tax. In addition, states that impose income and/or estate tax generally allow charitable deductions. An Income Tax Deduction is available for lifetime gifts to charity. The available deduction amount varies with: • The donor; • The type of charity; • The type of property donated; • The form of the gift; and • Whether the donor is an individual or a corporation. • Individuals to a public charity (church, hospital, university, etc.): • Cash is generally deductible up to 50% of the donor’s Adjusted Gross Income (AGI). • Ordinary Income property or short-term capital gain property is deductible up to 50% of AGI; however, the deduction is limited to the lesser of the donor’s cost basis or the property’s fair market value on the date of the gift. • Long-term capital gain property is deductible up to 30% of the donor’s AGI. (Alternatively, the donor can make a special election to deduct the lesser of cost basis or fair market value up to 50% of AGI.) • Gifts “for the use of” the charity are deductible up to 30% of AGI. • Note that partial interest gifts (i.e., those which are made into trust or which pay income only to the charity) may generate lower income tax deductions. • Individuals to a private foundation: • Cash is generally deductible up to 30% of the donor’s AGI. • Ordinary income property or short-term capital gain property is deductible up to 30% of AGI. • Long-term capital gain property is generally deductible up to 20% of AGI. • Contributions in excess of the above AGI limits may generally be carried forward to future tax returns for up to five years. Upon death, the five- year carry forward dies with the decedent, but it can be claimed on the decedent’s last return. • Corporations generally may deduct up to 10% of taxable income, with the five-year carry forward.

An Estate Tax Deduction is available for testamentary charitable gifts and is generally unlimited regardless of whether the recipient is a public charity or private foundation. However, partial interest gifts made at death are not eligible for a full deduction; only the present value of the charitable interest is deductible. Bridge to Healthy Families Program Continues to Help Caregiving Families By Soojin Yoo, MSW, Care Coordinator

It is often said that the caregivers are the unsung heroes of today’s long term care system for older adults. They take on this daunting task, usually with very little knowledge and experience, and do their best to be there and give back to their loved ones. Many caregivers find their experience to be meaningful and fulfilling, but the sad part is that caregivers often forget to pay attention to their own well-being because they are focusing all of their Soojin Yoo, MSW, Care time and energy on the Coordinator care they provide. The Bridge to Healthy Families program (BHF), which started at ACC in 2009 as a dementia care network, has been one of the leading resources for the caregiving families. BHF provides various types of support services, including care assessments and consultations, information and referral for services, educational workshops, support groups, and much more. Recently, BHF’s community nurse, Judie Higashi,

retired and ACC hired Soojin Yoo, MSW, as a Care Coordinator for the program. Soojin, a South Korea native, received her Master of Social Work degrees from Seoul National University in 1998 and University of WisconsinMadison in 2004 with a focus on family welfare. She originally joined ACC in 2012 and worked with the TEAM and CHANGES programs. Soojin envisions the BHF program to be a go-to resource for all the inquiries and services needs that caregivers will have throughout their journey. Any adult caring for an individual of any age with Alzheimer’s disease or a related disorder with brain dysfunction or any adult caring for an older adult (60 and over) requiring assistance with two or more activities of daily living (eating, dressing, bathing, walking, toileting, and transferring) is eligible for the services. ACC plans to make a special effort to make the BHF program more accessible to caregiving families with limited English proficiency by providing in-language support. The Care Coordinator will meet the families to conduct assessments and develop care plans. Follow up will be provided to make sure the family members access the necessary resources. This program is partially supported by a grant from the Area 4 Agency on Aging. Services are free with donations gladly accepted. For the Love of Food with Gerald Dumaguing

Gerald Dumaguing’s love of food began at home where he helped his mother and family in the kitchen. Growing up in a large family environment in Vallejo, food was always the center of all celebrations and events, and family always ate together at Gerald Dumaguing, Dietary the table. In 2005, Gerald Services Manager, at ACC Assisted Living and his family opened up a six-bed Residential Care Facility for the Elderly (RCFE) in Concord where he cooked the residents’ meals and assisted with caretaking and activities. During this time, he began attending the California Culinary Academy in San Francisco and went on to earn a degree in Culinary Arts

from Le Cordon Bleu. Gerald did his externship at Petals Restaurant in Benicia for six months. From 2007 to 2010, he worked at Bing Crosby’s Restaurant where he became the Head Pastry Chef after six months as Pastry Assistant. In the summer of 2010, Gerald joined the ACC Nursing Home as the Dietary Aide for a year and a half, and then transitioned to Activity Assistant for a little over two years. This year’s opening of Assisted Living at ACC Greenhaven Terrace provided Gerald with the opportunity to become the Dietary Services Manager. “I feel that everything I have experienced so far is coming in full circle and will help me contribute to ACC. I hope to share my love for all things food while still providing the excellent care and service that our residents deserve.” Gerald and his wife Heidi are the proud parents of two-year old Gavin. On their spare time, they enjoy eating, biking, gardening, watching movies, singing and playing music, and are very active with their church.

ACC Senior Services Friendly Visitor Program The Friendly Visitor program provides screened and trained Legacy Corps volunteers to help seniors and veterans retain their independence and assist the family and caregivers.

Want a Friendly Visitor? • Must be a senior • Have a family member who can complete a survey on caregiving Volunteers and seniors are matched based on shared interests, language, geography, and volunteer availability as much as possible. ACC Rides provides door-to-door transportation service to older adults. Priority is given to seniors without other means of transportation who wish to attend Meals on Wheels Café sites, programs and classes at ACC Senior Services Center, and other senior centers. Other services include rides to grocery stores, medical appointments, small group field trips, and other necessary errands.
