Dear Friends, On behalf of ACC Senior Services, we are pleased to present our very first Annual Report highlighting ACC’s achievements in this past year, and our vision moving forward.

We accomplished some exciting milestones in 2014, including the opening of Assisted Living at ACC Greenhaven Terrace with 24 apartments on the second floor. Adding this program supports our vision to provide a continuum of care for seniors. Meals on Wheels by ACC also delivered its two-millionth meal! The program continues to thrive, providing nutritious meals and regular friendly contact to thousands of seniors in Sacramento and west Placer counties. In April, ACC launched a vibrant new website that was designed to provide information on programs and activities in an easy and welcoming format. A fresh logo and new look accompanied the web design. As we grow our programs and services, we also have grown out of our space. Our year-end highlight is the completion of improvements of a 15,000 square foot space on Park City Drive across the street from ACC Greenhaven Terrace AND across the street from the current Senior Services Center (also on Park City Drive). This will be the ACC Campus, complete with large classrooms, staff offices, spacious meeting rooms, and room for ACC’s administrative functions. We look forward to welcoming you at the ACC Campus in the new year with more occasions for celebration, a “house warming,” and programs to better serve elders and their caregivers. All of these efforts are made possible through the dedication of generous donors, volunteers and staff. Our supporters wear many hats: staff members are often volunteers, and volunteers are also donors. When it comes to donations, we are extremely grateful that many of you give without being asked. Nearly 40% of all gifts received by ACC are unsolicited. We can’t thank you enough for supporting our mission to serve our community and enrich the lives of older adults. Sincerely,

The mission of ACC Senior Services is to promote the general welfare and to enhance the quality of life for our community by identifying, developing and providing culturally sensitive health and social services for older adults.


O U R S TO RY

ACC was founded in the late 1960s by leaders of the Asian American community to improve the quality of life for their elders in need of convalescent care and a place to get together. At that time, many seniors were experiencing social and language isolation in nursing homes, and they struggled with a lack of familiar foods and an inability to communicate with care staff. It led ACC to explore opportunities to provide quality care for individuals in need of placement. We also began to develop critical social services ranging from nutrition programs to cultural arts classes for seniors who were alone all day. In 1983, our first capital campaign was underway to build a nursing home. We also started operating bingo sessions in the community to raise funds. Four years later,

ACC opened what is now its Nursing and Rehabilitation Center. This continues to be our largest program, providing skilled nursing and rehabilitation services and a range of personal and supportive care. We continue to focus on the programs and service needs of a broad senior community. In addition, ACC has a special commitment to understand and address the needs of many Asian language groups in the region, including the development of specialized programs and resources. We take pride in our ability to welcome seniors of all backgrounds and ethnicities into the ACC family. Our programs have been recognized locally and nationally with numerous awards and distinctions for a high level of care and for our commitment to improving the quality of life of seniors and their family caregivers.

I N D E P E N D E NT LIVI N G AT ACC G R E E N HAVE N TE R R ACE At Greenhaven Terrace, residents enjoy the very best of both worlds. They can maintain their privacy and independence while having options to socialize and make new friends right outside their door. Residents and their families also have the peace of mind that the vast array of ACC Senior Services are always there for them. Our Independent Living community features 146 apartments with wonderful amenities including fully equipped kitchens, walk-in closets and private patios. Residents have access to the pool and spa, fitness room, library and computer room. They participate in a wide variety of classes and programs, including yoga, aqua aerobics and tai chi. Residents also organize evening bingo, bridge, mahjong, Scrabble and other pastimes in the community room. Transportation, housekeeping and meal services are available for residents who would like extra help that assures their well-being.

A S S I STE D LIVI N G AT ACC G R E E N HAVE N TE R R ACE Founded in 2014, Assisted Living is the latest addition to ACC and fulfills our vision of a continuum of services for older adults. Back in 2005, a survey of 600 seniors revealed a need for housing options, particularly for those with impaired health compromising their independence. ACC’s Assisted Living provides a welcoming, homelike environment that assures residents receive the individualized assistance they require.

I love it here!” — Amy Ishima, Resident ACC created the 24-unit Assisted Living wing as part of Greenhaven Terrace in order to create a senior community where residents can experience the benefit of increasing levels of care without having to move away from their neighbors and friends. For residents who need a little more day-to-day help, our Assisted Living option offers 24-hour supervision and assistance, three healthy meals, housekeeping and linen services. Activities are designed to promote opportunities for socialization and to meet the individual needs of each resident. Examples include escorted walks in the garden to build confidence and assure safe mobility; puzzles, mahjong and other table games; transportation assistance to shop and attend events; film nights; cooking or baking projects; and morning exercises that assure a good start to each day. 5

The residents are at the center of our care as we encourage them to make choices about their daily routines whenever possible. It is our belief that responding to individual preferences greatly improves the quality of life of residents. Every day, they are encouraged to select their own meals and dine in a social, home-like setting. It’s a revolution compared to the institutional tray service of earlier days.

Established in 1987, the 99-bed facility provides around-the-clock care with our licensed nurses on duty 24 hours a day, seven days a week. The ACC Nursing and Rehabilitation Center provides exceptional services including physical, occupational, and speech/language therapies, pharmacy, dental, vision, podiatry, laboratory and x-rays. All payment sources are accepted, including Medi-Cal, Medicare, and private pay.

A vibrant activity calendar features multicultural events, weekly religious services and engaging activities designed for group and individual participation.

The ACC Nursing and Rehabilitation Center is inspected annually by the California Department of Health Services and has an excellent survey history. We are known in the community for providing the highest level of care and customer satisfaction. Our staff takes great measures to build and nurture personal relationships with residents, their families and the community.

We’re redefining “active aging” with programs and educational opportunities promoting health and independence: • Bridge

to Healthy Families: In collaboration with the Alzheimer’s Association, we organized a network that improves access to comprehensive support services for family caregivers and care receivers. Services include a needs assessment and service referrals, home safety checks and support groups.

• Respite

Care: Innovative and affordable services for those in need of care so that family members or caregivers can get some respite. For example, through Legacy Corps, an AmeriCorps program, our Friendly Visitors volunteers allow family caregivers an opportunity to take a much-needed break as friendly companions to seniors or family members with disabilities in the family home.

• ACC

Rides: Door-to-door transportation to medical and dental appointments, shopping, field trips, Meals on Wheels Café sites and the ACC Senior Services Center. For many seniors, ACC Rides is a lifeline that provides a measure of independence.

Meals on Wheels by ACC: Fresh, warm meals are delivered to homes or served at 23 cafés throughout Sacramento and west Placer counties. Meals on Wheels is a non-profit subsidiary of ACC Senior Services, supported by government funds and public donations. In 2014, we hit a milestone, serving our two-millionth meal!

TH E VITALIT Y O F ACC

Volunteers Volunteerism is the foundation of our culture at ACC. We deeply value the dedication and support of hundreds of wonderful volunteers each year. Most of our volunteers wear many hats and may be as young as 16 or older than 99. Our staff, residents and their family members routinely give their time to support ACC programs, services and special events. We also welcome local students, professionals, homemakers, retirees and church groups serving in a wide variety of roles: as class instructors, activity assistants, entertainers, special event planners and helpers, meal servers and drivers. Volunteers also help ACC with community outreach, educational workshops and clerical and office support. In 2014, the ACC Volunteer Hall of Fame, which recognizes more than 20 years of volunteer service at ACC, had a total of 81 inductees.

Donors Donations, in any amount, make a difference. We are truly grateful to each and every one of our donors over the last six decades. Together, we have created and delivered programs and services to enhance the quality of life for our seniors today and for tomorrow. ACC also prides itself on operating with the utmost prudence in our financial undertakings. We are committed to using every dollar wisely, maintaining ACC’s fiscal health as we move forward.
