Reducing the cost of Occupational Health

Partnering with an Occupational Health (OH) provider comes at a cost, and in the current economic climate every business needs to look closely at the bottom line, however, an OH service in the UK can provide several economic benefits for both employers and the overall economy.

Here are some of the key economic advantages:

1. Improved productivity: OH services focus on promoting the health and well-being of employees. By addressing work-related health issues and providing preventive measures, OH services can help reduce absenteeism, presenteeism (where employees are at work but not fully productive), and workplace injuries. The growing cost of unattended OH appointments

With the emphases for many organisations being on the bottom line, it is no surprise that increased focus is being put on the cost of unattended OH appointments. These can be appointments the employee did not attend, or where consent was not obtained to proceed.

Potential costs for organisation can vary depending on various factors such as the size of the organisation, the nature of the appointments, and the specific circumstances. Looking at a cross section of PAM’s OH clients, we note that the amount of unattended appointments varied from 15% within the public sector to 18% within the private sector. In addition to the actual monetary cost of these unattended appointments, there are other impacts. These may be:

1. Lost productivity: When employees fail to attend OH appointments, it can lead to lost productivity. This can be due to the time wasted by the manager submitting the referral and the OH professional who could have seen another employee during that time.

2. Delayed return to work: OH appointments are often crucial for assessing an employee's fitness to work or determining suitable adjustments. If employees miss

these appointments, their return to work may be delayed, resulting in extended periods of absence and additional costs associated with hiring temporary replacements or redistributing workloads.

3. Administrative and time costs: Managing missed appointments requires managers time to have a meeting with the colleague, gain consent and resubmit the referral along with the administrative and triage resources required to make the new appointment. These time costs can add up, particularly in larger organisations where managers are managing numerous employees.

4. Impact on morale and employee well-being: Unattended appointments and instances where consent to proceed was not given by the employee, may indicate a lack of employee engagement or dissatisfaction, which can negatively affect morale and well-being in the workplace. This can result in reduced productivity, increased turnover, and potentially higher recruitment and training costs.

Ways to reduce these significant costs

Upskilling managers can play a crucial role in reducing the cost of unattended occupational health appointments. Here's why it's important:

Increased awareness and understanding: By upskilling managers they gain a better understanding of the importance of proactive healthcare management and the role that occupational health plays. They become more aware of the potential risks and hazards in the workplace, their impact on employee health and the support that is available. This awareness helps them identify early signs of health issues and take preventive measures, reducing the impact on potential long or short term absences.

Early intervention and

support: With enhanced knowledge and skills, managers can recognise signs of health issues among employees early on. They can encourage employees to seek timely medical assistance and provide necessary support, including referring to OH. Early intervention can help prevent minor health issues from escalating into more severe conditions, thereby reducing the potential for long term sickness absence.

They can educate employees about the importance and benefit of attending OH appointments and address any concerns or misconceptions. Creating a Menopause -Friendly Workplace

Over recent years there has been increasing awareness of the impact of the perimenopause and menopause on women or other people who have a menstrual cycle, this includes an increasing awareness of how it impacts on women both at home and in the workplace. 8 out of 10 women of menopausal age are in work. This is an increase on previous generations.

The menopause is a natural stage of life which affects most women and other people who have a menstrual cycle. This can include:

• Trans people – 'trans' is an umbrella term used to describe people whose gender is not the same as the sex they were assigned at birth

• People with ‘variations of sex development’ (VSD) – some people might also prefer to identify as intersex or use the term 'differences in sex development' (DSD)

It's important for employers to be aware of all of the people who might go through the menopause and menopause symptoms and to support them all equally. For the majority of women, the menopause will occur naturally between the ages of

45 and 55 with the average age being 51 for when there has been 1 year without periods. However, approximately 1 in 100 women will experience the menopause before the age of 40 and some women will have a medically induced menopause.

Common perimenopause and menopause symptoms include anxiety, mood swings, brain fog, hot flushes and sleep disturbances. In total there are over 30 recognised physical and psychological symptoms. Menopause symptoms usually last between 4 and 8 years. Though for some women symptoms will continue post menopause.

A survey by the British Menopause Society reports that for 45% of women menopause symptoms have negatively impacted on their work, and 47% who have had to take a day off work due to symptoms would not tell their employer the real reason for their absence. This could be for a number of factors including the age and gender of managers, a perceived lack of understanding from managers and embarrassment about the menopause from women effected.

Implement a menopause policy

With more and more women remaining in the workforce during menopause and beyond, it is important that organisations to implement a menopause policy. It will assist both managers and colleagues to know what support can be offered by their employer. It will also assist with the retention of staff.

Increase awareness and understanding of the menopause

Increase the awareness and understanding of the menopause for both managers and employees through training, talks and sharing of experiences. It will assist in building an open and supportive work environment for all.

Training for line managers

Having specific training for line managers will increase their confidence in being able to have a conversation about the menopause even if they are male and/ or younger than their colleague. They will be more confident in using the processes available to them within their organisation to support their staff.

Menopause support group

Set up virtual or in person support groups for women and other people who are going through the menopause so that they have a space to talk about how the menopause is impacting on them. It can be beneficial to have support groups where colleagues who are partners of someone experiencing the menopause have a space to talk about how it is impacting on them. This will assist all to be more open about their experiences and to create a more inclusive working environment.

Access to additional services

Having access to additional services to support women who are significantly impacted by the menopause will help to reduce absence and increase retention. Additional services such as PAM Wellbeing’s menopause assessment and 12 sessions of counselling can help to manage and support the psychological and emotional impact of the menopause and improve quality of life during this life transition.

BACK CARE AWARENESS WEEK

Young people’s disease

Back care awareness week is an important time for musculoskeletal clinicians.

Particularly around awareness weeks we should be spreading our knowledge to a wider community to give everyone a fair chance to understand common back conditions, what to do to support diagnosis, treatment, and effective selfmanagement.

This year, I would like to concentrate on Axial Spondyloarthritis, which is known as ‘young people’s disease’. I speak to a lot of young adults in my clinical practice, who have lived with the back pain for a number of years, yet they failed to receive the diagnostics and support they need and deserve to live their lives to the full.

There seem to be a common misconception that young people shouldn’t really have a back pain. And if they do develop one, it is because they are lazy, weak, or simply just strained or pulled a muscle. Sadly, this common misconception is widely present within the primary care providers, highlighting the need for training and education across the medical staff.

What is axial spondyloarthritis? (Axial SpA)

It is a chronic inflammatory type of arthritis that predominantly affects axial skeleton (spine and pelvis). Term axial spondyloarthritis is an umbrella term and it includes two categories:

Let’s have a closer look at Ankylosing Spondylitis or radiographic axial SpA Non-radiographic axial SpA

No changes on X-ray are present but inflammation is seen on MRI scan.

Changes to the spine or pelvic joints (sacroiliac joints) can be seen on an X-ray.

When it comes to a human body, nothing is ever straightforward. Statistics show that percentage of people do not present with inflammatory features on the MRI scan despite having the symptoms of back pain. This makes the condition even more challenging to diagnose.

Who does it affect?

It affects young individuals. Symptoms usually start in late teens to early twenties, with the average age of onset being 24. Very rarely people develop it after the age of 45.

Signs and symptoms of Axial SpA

Typical presentation includes:

• Gradual, non-specific (unrelated to trauma) onset of back pain

• Often alternating buttock pain

• Quite severe morning stiffness, which lasts longer than 30 min

• Pain occurring in the early hours of the morning, often waking the affected individual

• Pain eases with activity and increases with inactivity

If at least two symptoms are reported, then inflammatory back pain should be considered.

What about Ankylosing spondylitis?

In addition to back and pelvic pain and stiffness, ankylosing spondylitis almost always affects other body parts. During the assessment, it is worth establishing, if the individual has any problems with:

• Skin- such as psoriasis or other skin rashes

• Eye- vision problems, pain, or inflammation of eyes

• Bowel- abdominal pain, loose stools, irritable bowel symptoms

• Smaller joints- pain, swelling stiffness affecting joins of the lower and upper limbs.

• Fatigue, low grade fever, weight loss, generally feeling unwell.

Who or what is to blame for the young’s people disease?

As with many conditions, we don’t exactly know what causes this disease. Current research points that specific gene (HLA-B27) and environmental factors (infections, gut bacterial changes, toxins and pollutants, chronic stress) could lead to the onset of symptoms. We have to remember however, that the presence of the gene does not mean that the disease will develop.

And how to diagnose it?

As always, a thorough subjective assessment is the key. Blood testing (inflammatory markers and HLA-B27 gene testing) plus radiographic testing is also recommended. Due to complexity and difficulty in diagnosis of the condition, a referral to a specialist Rheumatologist is often necessary.

What about treatment?

• First of all- physiotherapy and regular exercise- to maintain the movement of the joints and muscle strength.

• To treat the symptoms (not the disease) of stiffness and pain caused by inflammation, non-steroidal antiinflammatory (NSAIDs) drugs are often used.

• In case of poor response to NSAIDs, corticosteroid injections or disease modifying anti-rheumatic drugs are used.

Use of biological medications, such as TNF and Biphosphonates can be considered.

WHAT ELSE SHOULD WE REMEMBER ABOUT?

• It’s a lifelong disease, therefore hands on approach to treatment is essential.

• Eating well- research shows that antiinflammatory type diet can significantly reduce the inflammation, thus symptoms.

• Smoking does not benefit people living with Axial Spa. This is because smoking can reduce effectiveness of prescribed drugs. If the disease affects the ribcage, making it stiff and not expandable, then any smokerelated lung issues (COPD) will feel worse.

• Mental Health can be severely affected by the disease, particularly when it affects really young individuals. Research shows that 59% of people with axial SpA (AS) report experiencing mental health problems. Consider CBT Therapies or Chronic Pain Management Programs to get control of mental health.

• Seeing Occupational Therapist for supportive aids might be required, particularly during flares.

• Speaking to Occupational Health Specialist for advice on Workplace Adjustments.

Promoting Workplace Wellness: Thinking about Suicide Prevention

In 2021 there were 5,583 registered deaths by suicide in England and Wales, equating to an average of 15 suicides per day [Source, ONS].

Suicide is a deeply impactful and upsetting issue that affects individuals from all walks of life, and the workplace is no exception. Given the significant amount of time employees spend at work, the workplace can have a substantial impact on mental health.

As responsible employers and colleagues, it is crucial to prioritise suicide prevention in the workplace. By fostering a supportive and understanding environment, we can play a vital role in promoting mental wellness and preventing tragic outcomes. We explore five ways in which the workplace can play a part in suicide prevention.

1. Raise Awareness and Reduce Stigma
