EDITORIAL

What does 2024 have in store for us?

Since 2020, my philosophy for the next year is not what am I going to accomplish, but what am I going to have to deal with. When the world is out of sorts, how do we deal with the constant unknown and unplanned? As dentists, we are constantly dealing with the unknown and the unplanned. Not for

a lack of planning, but many days I feel more like MacGyver defusing a bomb in the patient’s mouth than someone restoring their dentition. Being open to analysis and change is paramount in our field. Utilizing this mentality for 2024 could adjust a volatile year to smooth sailing.

To quote Ted Lasso, quoting Walt Whitman, “Be curious, not judgmental.” Full disclosure, I have not watched Ted Lasso completely. (Like most things, it is on my list of things to do.)

I did however watch this clip while scrolling through social media (which is not on my list) and it perfectly encapsulates the mantra of a good leader.

Forbes takes this scene and breaks it down to the power of asking good open-ended questions. Many dentists have mastered the art of the open-ended question. Assessing a patient’s chief complaint is a master class in open-ended questions and refining our analysis of the situation.

The curious clinician knows that getting the patient to talk helps get to the "root" (see what I did there?) of their concern and the better we are able to move forward with a plan that addresses their need(s). Getting the patient on board with treatment is a practice in curiosity

without judgement. Finding the patient’s “why” and the “what” is the difference between “not right now” and “let’s do it."

“Be curious, not judgmental” could also be a philosophy for how we approach people and problems. The disgruntled patient in the waiting room, the assistant not able to make it to work on time, a disagreeable family member are all instances where curiosity could resolve the conflict. When we are open to how the other person perceives the situation outside of our judgements, it allows for resolutions. Leaders seek resolution, not conflict. Leaders ask the tough questions that need to be asked. Leaders address the issues others avoid.

Understanding the whole issue is paramount to extracting (I did it again) the cause.

So, for 2024, let’s bring the curiosity! Let us ask the questions and explore the options. Face the challenges with resolution and insight while leaving the judgement in 2023 where it belongs! As leaders in our home, office, and community, it is within our means to set the tone for the year ahead and that can begin with a simple question:

Corporate Transparency Act requires most practices to report ownership

provided by the American Dental Association

The deadline for filing beneficial ownership reports is Jan. 1, 2025.

Dental practices that meet certain requirements can begin providing information on both the practice and its “beneficial owners."

The Corporate Transparency Act, enacted by Congress in 2021, intends to combat the use of businesses as moneylaundering operations. It requires certain businesses to report information to the U.S. Department of the Treasury’s Financial Crimes Enforcement Unit about their ownership. The Financial Crimes Enforcement Network began accepting beneficial ownership information reports on Jan. 1, and all existing dental practices and companies that meet the requirements have one year to file before facing penalties.

Reporting companies created or registered to do business in the U.S. before Jan. 1, 2024 must file by Jan. 1, 2025.

Dental practices must file under the act if it meets one or both of the following criteria:

The practice employs fewer than 20 people.

The practice generates less than $5 million (gross receipts) in revenue annually.

Failing to file a report, knowingly providing false information, or refusing to provide information if you are a beneficial owner can all carry both civil and criminal penalties. Civil penalties include fines of up to $500 per day until the violation is fixed. Criminal penalties include fines of up to $10,000 and/or imprisonment for up to two years.

According to the statute, a “beneficial owner” need not necessarily own shares or have a financial stake in the business. A beneficial owner:

Owns or controls at least 25% of the business, or Exercises substantial control over the business.

The final rule clarifies that “substantial control” includes senior officers of a business, as well as anyone with significant influence over important decisions, even if that person has no formal decision-making power. That also extends to any contractual or financial relationships.

Beneficial ownership information reporting is not an annual requirement. A report only needs to be submitted once,

unless the filer needs to update or correct information. Existing dental practices will need to report information on both the practice itself and its “beneficial owners.”

Employees of a dental practice need do nothing unless they are significantly involved in making business or financial decisions for the practice. In that case, the employee may be asked to provide their information as a beneficial owner.

A letter from the ADA to the U.S. Senate was sent the week of Jan. 8 urging them to pass legislation extending the deadline for companies to report ownership information.

GET INVOLVED Volunteer Opportunities with ODA

Are you interested in becoming more involved in the Oklahoma Dental Association? Are you interested in developing your leadership skills and representing your colleagues at the state level?

There are many opportunities for ODA members to volunteer their time, advocate on behalf of the group, support charitable dentistry, plan our largest events, and to serve in other leadership roles within the organization. A few of the options include:

BOARD OF TRUSTEES

This is the administrative body of the Association and is comprised of the elected and appointed officers of the association, delegates to the ADA, one trustee elected by each of the seven components, and a new dentist trustee.

HOUSE OF DELEGATES

Establishes all ODA policies not otherwise provided for in the Bylaws, elects the Speaker of the House and council members, confirms the appointments of the Secretary/ Treasurer and the council chairs, adopts an annual budget, and sets the membership dues. The House of Delegates meets annually and is comprised of the Board of Trustees, four officers from each Component, one delegate per fifteen members from each Component, and four OU College of Dentistry students.

ANNUAL MEETING PLANNING COMMITTEE

Coordinates all aspects of the ODA Annual Meeting, including the selection of scientific speakers and topics, procuring sponsorships, and planning social functions.

OKLAHOMA MISSION OF MERCY (OKMOM) PLANNING COMMITTEE

This free dental clinic event is divided into many areas, both clinical and non-clinical, with each area being coordinated by a “Lead” volunteer. This committee oversees the planning and onsite execution of the event. All OkMOM Leads must be available for set up, tear down, and both treatment days of the event. Next year's event will take place in Tulsa, OK on February 7 and 8.

COUNCIL ON BYLAWS, POLICY, AND ETHICS

Annually reviews the bylaws and all policies of the Association, as well as investigates, mediates, and adjudicates complaints regarding a member’s alleged non-compliance with the ADA’s Guidelines and Principles of Ethics and Code of Professional Conduct.

REWARDS PARTNERS COMMITTEE

Enhances membership value by seeking out and securing relationships with companies that may offer discounts on professional and personal products and services exclusively for ODA members while generating non-dues revenue for the association.

EDITORIAL COMMITTEE

Creates and solicits content for the ODA Journal and conducts dental health education campaigns and programs to improve the public perception of dentistry.

COUNCIL ON BUDGET AND FINANCE

Develops an annual budget for approval by the House of Delegates and oversees the annual audit.

COUNCIL ON GOVERNMENTAL AFFAIRS

Coordinates lobbying activities and maintains a “grassroots” program for effective communications between ODA members and their legislators. Interacts with state and federal boards and agencies and develops recommendations concerning rules, regulations, or legislation that affect the dental profession or the dental health of the public.

COUNCIL ON MEMBERSHIP AND MEMBERSHIP SERVICES

Develops all membership recruitment and retention programs and assists with outreach and calling campaigns to new and potential members.

DENPAC BOARD OF DIRECTORS

Oversees the Oklahoma Dental Political Action Committee (DENPAC) which works to support the dental profession by making political contributions to dentistry-friendly candidates running for state-level elected office.

DENTAL RELIEF AND DISASTER GRANT PROGRAM BOARD OF TRUSTEES

Oversees the Oklahoma Dental Relief and Disaster Grant Program, a charitable trust established by dentists to provide aid to dental professionals affected by natural disasters, physical disability, chemical dependence, or other hindering conditions.

2024 ODA Annual Meeting SPEAKERS

Parul Dua Makkar, DDS

Irene Iancu, RDH, CTDP

Dr. Parul Dua Makkar completed her Bachelor of Science from the University of Central Oklahoma in 1999, Magna Cum Laude, and then attained her DDS from the University of Oklahoma College of Dentistry in 2003. She completed a GPR training at Staten Island University Hospital in 2007 and has been in private practice in New York since. She owns PDM Family Dental in Long Island, NY, where she resides with her husband and two boys. Dr. Makkar’s life took a different trajectory when she lost her only and younger sibling, Dr. Manu Dua, to oral cancer last year. He was a dentist, as well. Since his death, she devotes her time educating doctors and patients alike about risk factors, prevention and advocating for early diagnosis of oral cancer. She has co-authored several dental journal articles, presented lectures to dentists, has been a guest on several podcasts, and hosted her own podcast. She believes oral care is the gateway to overall wellbeing and aims to have open conversations with her patients. She is currently serving as a Wellness Ambassador to the American Dental Association and is a Fellow of the American College of Dentists.

Irene is a practicing dental hygienist and orofacial myologist who founded and owns Toothlife Studio in Toronto. She is a theoretical and clinical educator at Oxford College, international speaker, author, podcaster, and social media expert. Irene is a peer and quality assurance mentor contracted by the College of Dental Hygienists of Ontario, a writer for The National Board of Dental Hygiene Examination, and a regular contributor to Oral Hygiene Magazine. She originally graduated with a political science degree but changed course after completing the dental hygiene program in 2007 from Oxford College. Her greatest passion is working with children and orthodontists to retrain facial muscles in order to break damaging habits.

2024 ODA Annual Meeting SPEAKERS

Dr. Melissa Seibert is a dentist proudly serving in the U.S. Air Force and is the creator and host of the top clinical dental podcast, Dental Digest, which helps dentists to stay on the cutting edge of evidence-based dentistry. Dr. Seibert is the deputy program director of the AEGD at Langley AFB and Clinical Instructor at Creighton University School of Dentistry. She is a key opinion leader, consultant, and author of numerous articles, and she lectures to national and international audiences. Her areas of interest include restorative dentistry, adhesive dentistry, digital dentistry, and prevention. Dr. Seibert founded the Advanced Interdisciplinary Mastermind – a study club comprised of top-tier clinicians. She received her Doctorate of Medicine in Dentistry at the University of Louisville. Prior to that, she received her Bachelor of Science degree and commission as an Officer in the United States Air Force from the U.S. Air Force Academy.

Dr. Jennifer Jenkins and Dr. Sara Spurlock became friends at the University of Oklahoma College of Dentistry. While their paths initially went in different directions, in 2014 Jennifer joined Sara in Norman and they began cultivating their dental practice. Both doctors are committed to advancing their profession through science and have achieved this by dedicating themselves to continuing their education. In 2022, Dr. Jenkins and Dr. Spurlock completed all nine courses from the world-renowned Kois Center in Seattle, WA.

Their experience and knowledge gained at Kois enabled them to implement a successful periodontal program, and they have found fulfillment in partnering with patients to effectively treat periodontal disease and realized genuine success in improving the health of their patients. It was at the Kois Center where their passion for oral integrative medicine was ignited.

In their free time, both doctors have obligations to their profession serving as President and Vice President of the Central District, co-lead triage for the Oklahoma Mission of Mercy, and serve on boards for the Oklahoma Dental Association and Board of Dentistry. Dr. Jenkins is active in the Sooner Rotary Club of Norman, OK serving as president during 2022-2023, and Dr. Spurlock serves on the Norman Regional Hospital Foundation Board. Both doctors serve on the Children’s Hospital Foundation Board.

In addition to dedication to their patients, practice, community, and profession, Drs. Jenkins and Spurlock are dedicated to their families. Dr. Jenkins and her husband, Justin, have two children, Jase and Jax. Dr. Spurlock and her husband, Jay, have three children Anna, Will, and John Henry.

2024 ODA Annual Meeting SPEAKERS

Dr. Dunn Cumby attended the University of Oklahoma for his undergraduate degree and continued on to earn his MPH and DDS, also from the University of Oklahoma. He was the first black man to graduate from OU’s College of Dentistry in 1976 – the same year he founded his private dental practice and became a faculty member at the school. Dr. Cumby worked as the division head in community dentistry at the University of Oklahoma College of Dentistry for more than 43 years and oversaw the Community Dental Health Coordinator Pilot Training Program. Dr. Cumby sold his practice and retired from full time work in 2019, but he continues to teach Introduction to Ethics at the College of Dentistry today. In addition to his dental career, Dr. Cumby is an ordained deacon for the Catholic Archdiocese of Oklahoma City, a 4th Degree Black Belt, and a World Taekwondo referee.

Eric Hopkins has been performing general dentistry in Shawnee, Oklahoma since receiving his DDS from the University of Oklahoma in 1989, and he has been performing advanced dental implant procedures since completing the AAID Maxicourse in New York, NY in 1999. He achieved board certification with the ABDS in 2018 following successful completion of the oral and written examinations for the American Board of Oral Implantology/Implant Dentistry (ABOI/ID) the same year. Dr. Hopkins is a Diplomate of the ABOI and a Fellow of the American Academy of Implant Dentistry. He teaches various aspects of implant dentistry, from simple to complex, through Hopkins Implant Institute as well as the L-PRF class for accreditation of dental auxiliaries, and serves as the Director of the AAID Maxicourse Oklahoma City.

Dr. Brian Baliwas received his undergraduate degree in Biochemistry and Molecular Biology from University of California Davis, followed by his doctorate in dentistry from the University of the Pacific in San Francisco, where he graduated with high honors. He is also a graduate of the Kois Center. He has received several awards for his outstanding clinical work in the areas of esthetic dentistry, oral implantology, restorative dentistry, and endodontics. Dr. Baliwas teaches part-time at the University of the Pacific Dental School in the Department of Integrated Reconstructive Dental Sciences, where he inspires his students to maintain a high level of commitment, passion, and pride in their dental work. She serves as a mentor for the University of Oklahoma College of Dentistry and is a member of the American Dental Association, Oklahoma Dental Association, and the Oklahoma Society of Oral & Maxillofacial Surgeons. Dr. Lunday is a clinical assistant professor for the department of oral and maxillofacial surgery at OU COD and the OMS residency training program. Dr. Lunday is active staff at Baptist Medical Center, Mercy Hospital, and OU Health as well as serving on various committees for the Board of Dentistry. Kelly McClure, RDH

Kelly McClure graduated from Coastal Bend College with a degree in Dental Hygiene. She has practiced hygiene for 23 years with the mission to establish trust with her patients and create systems and protocols to grow the hygiene department. Kelly, along with two other hygienists, co-founded a consulting company called Hygiene Solution to help doctors and practices understand their potential and increase revenue without compromising patient care. After her time with Hygiene Solution, Kelly joined Align Technology as a Digital Adoption Specialist. In this role, she works with teams to implement the Align Digital Platform and is particularly passionate about helping dental professionals transform their approach to patient education.

Kelly and her husband, Steve, have two children, Abby and Jackson. She enjoys spending time with family and friends, reading, and cheering on the University of Arkansas Razorbacks, where her daughter Abby recently started attending college.

Ashley Sutherland grew up in northwestern Oklahoma, and at a young age she knew she wanted to pursue a career in the dental field. After high school, she pursued her passion by enrolling in the dental assisting program at Platt College and has now been in the dental field for over 18 years. During that time, she became an iTero mentor with Heartland Dental where she taught other offices how to educate their patients on the importance of health and occlusion. Because of her expertise in scanning, she became a DA mentor for two years before joining Align Technology to continue her passion of helping dental teams fully implement digital scanners. She loves spending time with her nieces and nephew, walking her dog, and traveling as much as possible.

Please note: ODA’s Annual Meeting CE credits are not PACE or CERP certified, although they are compliant with all rules governing those programs. The Oklahoma State Board of Dentistry has pre-approved all ODA Annual Meeting courses, and courses will count toward continuing education for licensure in the State of Oklahoma, but may not be applicable toward national level fellowships or other programs. Each attendee will be asked to complete an online survey after the event to receive their CE certificates.

LEGISLATIVE LOOP

National Level Negotiations Create Temporary Setback

The first session of the 59th Legislature (Spring 2023) resulted in a failed attempt to establish and enforce a Dental Loss Ratio (DLR) for Oklahoma (the dental insurance premium revenue that is spent on patient care instead of operating costs).

Rep. Marcus McEntire (R-50) was successful in passing HB 1694 off the House floor with a vote of 77-0, but as the bill transitioned to the Senate, it met resistance and failed on a 5-5 vote in the Senate Insurance Committee.

Already in 2024, nine state dental societies have introduced legislation for dental loss ratios. Along with Oklahoma, Illinois, Nebraska, New York, Pennsylvania, Rhode Island, Virginia, Washington, and West Virginia have also introduced DLR legislation. Arizona, Colorado, and Nevada adopted legislation in 2023, and New Mexico adopted legislation in 2022.

The tie vote in the Senate Insurance Committee in 2023 was considered “final action” which then also triggered the application of certain rules within the Senate. For the past several months, we have been working to overcome the impact & interpretation of Senate Rule 6-7 which states:

“If final action is such as to defeat an amendment, a bill, or a resolution, no other amendment, bill, or resolution having the same effect and covering the same specific subject matter shall be considered by the Senate during either session of the current Legislature, unless otherwise approved by the President Pro Tempore.”

Our Senate author Paul Rosino (R-45), who has been a friend of the profession, had multiple conversations with Senate staff and leadership. Even though we had made substantial changes to our 2023 bill language, Senate staff was very direct with their interpretation which ultimately influenced the Senate’s position not to allow the bill to move this session.

While these conversations were taking place, the National Council of Insurance Legislators (NCOIL) voted at their January 26 meeting to pass model legislation for a dental loss ratio. The bill includes language negotiated between the ADA and the National Association of Dental Plans, which represents dental insurance and third-party payer companies. The NCOIL model language, which was immediately shared widely by the insurance industry, does not go nearly as far as the ODA bill in protecting our patients. The unfortunate timing of this announcement and the “perceived compromise” was undoubtedly the final blow to any hope for this session with regards to running successful DLR legislation.

While the Senate rule is certainly a frustrating reality, the conversations with legislators our lobby team has had throughout the interim has been very positive. The ODA has gained ground on this issue with legislators, but the NCOIL language provided a very “convenient out” for those legislators who were unsure of their position. The insurance industry will use this model language as a way to drive a wedge between our lawmakers, negatively affecting any additional conversation this session about DLR in Oklahoma.

Despite the temporary setback, the motivation to protect our patients by ensuring dental insurance premiums are directed toward actual care is an endeavor worthy of pursuit even if it takes another year.

In a democratic society, citizen participation is not just a privilege but a fundamental right. While voting during elections is a cornerstone of democracy, true engagement extends beyond the ballot box. One of the most effective ways for individuals to influence policy decisions and enact change at the state level is by contacting their state legislators. This direct communication channel serves as a powerful tool for citizens to ensure their voices are heard when critical decisions are being made.

Contacting state legislators provides a direct line of communication between constituents and their elected representatives. State legislators are elected to serve the interests of their constituents, and by reaching out to them, individuals can express their concerns, opinions, and suggestions on various issues affecting their communities. Whether it's advocating for better oversight, addressing healthcare disparities, or supporting initiatives that are positive for the profession of dentistry and for your patients, reaching out to your legislators enables citizens to articulate their priorities and hold elected officials accountable for representing their interests.

Talking with your state legislators also fosters civic engagement and promotes a sense of ownership among citizens. When individuals actively participate in the democratic process by engaging with their elected officials,

they become more invested in the decisions that shape their lives and communities. This involvement not only strengthens the process but also empowers individuals to take an active role in shaping the future of their state. Your efforts can have a tangible impact on policymaking. Elected officials rely on feedback from constituents to inform their decisions and draft legislation. By expressing their views and providing relevant information, citizens can influence the legislative agenda and shape the direction of public policy. While the political landscape may be complex and multifaceted, the collective voices of engaged citizens wield considerable influence in driving positive change and addressing pressing issues.

ACTION FOR DENTAL HEALTH (ADH) PROGRAM

Continued to lobby for more funding toward the ADH program, which provides federal funding through the HRSA State Oral Health Workforce Grants to address the dental health needs of underserved populations.

DOC ACCESS REFORM

Continued support advocating for the Dentist and Optometric Care Access Act or the DOC Access Act in Congress to foster insurance competition, benefit consumers, and bring balance to contract negotiations. MEDICAID REFORM

The ADA develops resources that state dental societies have used to reform their Medicaid programs, making it easier for dentists to participate and deliver patient care. Federal advocacy continues to ensure states provide comprehensive dental services to adult Medicaid beneficiaries.

THE DENTIST AND DENTAL HYGIENIST LICENSURE COMPACT

Three states have passed legislation designed to allow dentists to practice in multiple states (WA, IA, and TN), three more have active legislation for 2023 (OH, PA, NJ), and nearly a dozen states are expected to file this legislation in 2024. Seven states total must pass the DDH Compact before it becomes effective.

Report from the ODA Board of Trustees

The Board of Trustees met on November 3, 2023, to conduct regular business of the ODA.

The Board recommends that the bylaws and policy be amended by designating the ODA President-elect as the Secretary of the ADA Delegation. Currently the ODA Vice President serves as the Delegation Secretary. That person attends 12th District meetings and is reimbursed for those meetings by the ODA, but has no vote on the ADA House of Delegates and does not serve on ADA reference committees. The ADA bylaws do not dictate how states determine their delegations. The change would save the ODA a minimum of $3,500 per year and allow the Vice President more time in their office.

Upon recommendation by the Council on Budget and Finance, the Board adopted the 2024 operating budget, without a dues increase, with projected income of $1,159,513 and projected expenses of $1,158,371, resulting in a net of $1,142. The budget will be submitted to the House of Delegates in April for ratification.

The Board voted to change the group insurance endorsement for members from New York Life Insurance to Met Life Insurance. The ODA shares a group insurance plan, administered by 3000 Insurance Group, with the Oklahoma Bar Association and the Oklahoma Society of CPAs. 3000IG recommended replacing New York Life Insurance Company with Met Life Insurance Company to handle the endorsed benefits of Disability Insurance, Business Overhead Expense Insurance, and Life Insurance. The existing relationship with New York Life has grown stagnant, with very little support from New York Life. The goal of this change is to:

• Increase some current certificate holder benefits

• Decrease some current certificate holder premiums

• Improve marketing support

• Improve underwriting service

• Allow this book to grow so it will become more stable and predictable

The Board amended the current check signing policy so that checks over $5,000 and checks written to Lynn Means can be signed by ADA Delegates, in addition to any ODA officers, to increase availability of local check signers.

The Board recommends sunsetting the Councils on Dental Care and Dental Education and Public Information. One of the objectives in the current ODA strategic plan is to evaluate all ODA Councils and Committees to determine effectiveness, redundancy, and relevance to maximize member engagement. Current policy states that the Councils are required to meet at least annually. The Councils on Dental Care and Dental Education and Public Information typically do not have business to conduct, nor can they rarely confirm quorum. Both Councils mainly convene to hear reports from staff or allied organizations, which can be obtained and reported to the Board of Trustees and House of Delegates directly. Special task forces may be appointed by the President, as needed, to address specific issues by members with those special interests or expertise to maximize member engagement.

The next meeting of the Board of Trustees is scheduled for March 1, 2024, at 1:30 p.m. at the ODA building in Oklahoma City. Any member with questions or concerns for the Board may contact their Component Trustee.

Rare Disease on the Rise

UPTICK IS SIGNIFICANT FOR OKLAHOMANS' HEALTH

This case report carries noteworthy implications for oral healthcare providers in Oklahoma, addressing a rising health concern within the state. The knowledge gained from this case can empower practitioners to recognize critical signs enabling early diagnosis and intervention. The goal is twofold: to prevent the transmission of infection, and also to minimize the risks of severe morbidity in individuals with early treatment of this disease.

An elderly gentleman presented with the following timeline of events and photos of oral lesions. The areas caused mild discomfort when eating certain foods but not extremely painful. The patient had a history of smokeless tobacco and alcohol use.

April 2023: Patient presented with persistent “canker sores” to primary care physician twice and was treated unsuccessfully with antibiotics and allergy medications.

May 2023: Routine dental appointment revealed red and white, flat lesions on the tongue and lower labial mucosa. Referral was made by his general dentist to an oral surgeon for biopsy and treatment.

July 2023: General dentist notices the lesions have evolved significantly; patient returned for other treatment. Biopsy appointment was set for mid-August.

August 2023: Biopsy performed from right lower lip and surgeon described the areas as erythroleukoplakias. The biopsy diagnosis was hyperkeratosis, acanthosis with acute and chronic inflammation. A special stain for fungal organisms (candida) was performed and was negative. This initial biopsy specimen was not evaluated by OU Oral Pathology Lab at this time.

October 2023: His general dentist notes the persistence of the lesion and refers the patient to OU Oral Pathology.

November 2023: Evaluation by oral pathologist and recommendation for certain tests to be ran by primary care physician to rule out a certain infectious disease.

What is the suspected disease associated with this history of present illness and clinical presentation?

CASE STUDY

It is important to appropriately describe the appearance of these lesions to accurately establish a potential diagnosis. It is easier said than done in this instance because at first glance the lesions could be seen as ulcers. However, upon closer examination these lesions are raised areas of oral epithelium creating a plaque, or a patch of tissue. Although there is a faint yellowish hue to the affected areas, it is essential to note that this coloration does not arise from fibrin, the characteristic protein associated with most ulcers. Instead, this is a plaque of epithelium harboring exuberant numbers of neutrophils, imparting a subtle yellow tint. Neutrophils, the tiny vigilant first responder cells of the immune system, are particularly adept at combating bacterial infections. Indeed, these lesions are due to a bacterial infection seen here.

Treponema pallidum to be exact - better known as Syphilis or Lues.

Syphilis is a bacterial infection that is known to only occur in humans. The bacteria are primarily spread through sexual contact or from mother to fetus. The infection has three stages with primary syphilis usually presenting within 3 to 90 days at the site of inoculation as a chancre or a non-painful ulcer. As you look back at the case, this was probably the “canker sore” described by the patient when he reported it to the primary care physician. Most chancres occur in the genital regions suggesting a sexually transmitted disease. However, 4% of chancres occur in the oral region and most commonly on the lip, but other sites may be affected including the buccal mucosa, tonsils, tongue, and keratinized tissues. The initial lesion will heal on its own with no treatment in just a few days.

In the secondary stage of syphilis, typically appearing 4 to 10 weeks after the initial infection, systemic symptoms may manifest, and there can be overlap between primary and secondary lesions. Patients might exhibit painless lymphadenopathy, a sore throat, fever, headaches, or musculoskeletal pain. A hallmark sign of secondary syphilis is a diffuse, painless, maculopapular rash that can appear on various parts of the body. It is crucial to recognize that these non-specific symptoms were absent in our case, emphasizing that their absence doesn’t exclude syphilis as a potential diagnosis, especially when dealing with white plaques in the oral cavity. Oral healthcare providers most often encounter syphilis cases during the secondary stage, with mucous patches being the prevalent oral finding. These lesions present as sensitive yellowish-white elevated plaques, as observed in the current case, triggered by an intense inflammatory response to the spirochete infection. Commonly, mucous patches are found on the tongue, lip, and buccal mucosa, and multiple lesions are typical during this stage.

Although spontaneous resolution typically occurs within 3-12 weeks, it is imperative to initiate treatment before resolution to prevent false negative testing during the latent phase or potential organ damage in tertiary syphilis. Infection can remain latent for 1 to 30 years before possibly progressing to tertiary syphilis. It is essential to recognize that patients may more readily test negative for syphilis during the latent period.

It's essential to understand that syphilis is highly infectious during the initial two stages. Individuals with oral lesions can unknowingly transmit the bacteria through everyday activities, like kissing. Therefore, a delayed diagnosis and treatment could have unintended consequences on the well-being of a family, community, or even the broader state. This specific case illustrates how a person could have highly infectious areas in the mouth for an extended period, all while remaining undiagnosed by medical doctors and oral health specialists.

Tertiary syphilis may develop in approximately 30% of cases after a period of latency. Tertiary syphilis represents the most severe phase of the infection and is potentially life threatening. The vascular system and central nervous system can be targets of the infection resulting in severe conditions like aortic aneurysms, congestive heart failure, neurologic disorders, general paralysis, and psychosis. Oral lesions that may arise in tertiary syphilis include gummas, a granulomatous inflammatory condition that can also affect the skin and other internal organs, bone, and soft tissue. While syphilis is treatable and curable in its early stages with sufficient antibiotic therapy, the harm inflicted on organs due to syphilitic infection is often irreversible.

The diagnosis of syphilis can be somewhat challenging due to several factors. The Venereal Disease and Research Laboratory (VDRL) and rapid plasma regain (RPR) can test negative for several weeks following initial infection. This is due to delay in antibody production by the patient. However, after 6 weeks this can be a reliable test and recommended - but beware of early false negatives early in the disease process. Biopsy and dark-field examination on a smear can be challenging to confirm infection with Treponema pallidum since other treponema organisms can be isolated from the oral cavity. However, these latter methods could be most helpful in making the earliest possible diagnosis due to the delay in antibody production for serologic studies. There is a specific immunohistochemical marker for T. pallidum which can be helpful to make an early diagnosis if suspicious for the infection. Serologic testing remains the gold standard for diagnosis.

The incidence of syphilis infections is on a notable rise in the

United States. According to CDC data, there was a substantial increase from 13,970 reported cases of primary and secondary syphilis in 2011 to 53,767 cases in 2021, marking a 3.8-fold surge over the past decade. A closer examination of the situation in Oklahoma reveals even more alarming statistics. In 2011, the reported rate stood at 2.2 cases per 100,000 population for primary or secondary syphilis, and by 2021, this rate escalated to 30.7 cases per 100,000 population. This ranks Oklahoma fourth nationally for the highest rate of reported primary and secondary syphilis cases per 100,000 population, following South Dakota, New Mexico, and Arkansas. While various factors contribute to the heightened incidence in the United States, oral health care professionals can play a crucial role in mitigating this trend by fostering early awareness and recognition of oral lesions. This proactive approach holds the potential to curb the transmission and morbidity associated with this insidious disease.
