The year 2018 was filled with accomplishment for Ochsner Health Network (OHN) and in this, our second report since OHN’s inception, we are pleased to share those accomplishments. OHN is a physician-led network committed to improving population healthcare, and in 2018 we found new ways to serve and grow. Metrics help tell the story, including 55 percent growth in covered lives, now exceeding 245,000; a 16% increase in the number of physician practices participating in our network; numerous metrics that indicate patients treated by OHN providers remained healthier and better managed their chronic diseases; $30 million earned in value-based payments; and $1 billion in healthcare spending now managed by OHN across the state of Louisiana. These statistics are important, but there’s more to the story. The way we truly measure ourselves is by the value we create for OHN’s stakeholders. After all, delivering stakeholder value is OHN’s reason for being, as we seek to leverage our scale and expertise through collaboration, evidence-based protocols and powerful analytics to deliver a higher level of care at a lower cost. The healthcare landscape is changing in fundamental ways – and it is happening quickly. OHN’s stakeholders are both driving change and being impacted by it. Physician providers are increasingly facing the challenges of digital disruption, pricing transparency and the growing number of quality measures to which they are held accountable. Employers are looking to networks to create value by lowering costs, creating better experiences for their employees and keeping their total medical expenditures down. Payers are seeking alignment with provider networks as they try to match payment with performance. Patients want healthcare that’s more affordable from providers they trust. Finally, government leaders and regulators aim to improve healthcare outcomes across our state, which, today, lag the nation. These are diverse and compelling needs and OHN is working to play a bigger role in meeting them. It starts with an even closer alignment with our providers – the physicians and group practices with whom we partner. They face disruption from new entrants into healthcare that seek to create a different care model, combining brick-and-mortar and digital solutions. And there are also opportunities – like leveraging artificial intelligence in new ways and deploying predictive models that get ahead of healthcare crises and intervene to improve outcomes. OHN’s objective is to help providers navigate this landscape, and continue to deliver the knowledge, advanced capabilities and support infrastructure providers need to be competitive in value-based care delivery. OHN was founded as a combination of networks comprised of six of the leading healthcare providers in the state. We’re pleased at the value we’ve been able to deliver so far – reducing costs, helping providers be more competitive and profitable, and improving health outcomes. But the story of disruptive change across healthcare is not over – in fact, it is just beginning. As we look ahead, OHN must continue to examine our structure and processes to identify new and more impactful ways to deliver value. Collaboration underpins all we do at OHN. We are grateful to our partners and members who work collaboratively to achieve the objectives we all seek – higher quality and more cost-effective care that benefits our entire community. We thank them, and all our stakeholders, for their continued engagement and support.

2018 was a year of significant growth and accomplishment for Ochsner Health Network. Among the most significant milestones:

Improved outcomes for Blue Cross patients with hypertension by controlling blood pressure for 75% of patients with the disease.

Increased the number of at-risk lives cared for by OHN physicians by 55% to more than 245,000.

BUILDING ON SUCCESS

St. Tammany Physicians Network St. Tammany Parish Hospital (STPH) has been delivering quality healthcare for 65 years. In 2014, STPH wrote a new chapter in its history through its partnership with Ochsner Health System. The goals of the new partnership: increase local access to care, improve quality and reduce costs for area patients.

STPN Quality staff engages providers by analyzing, presenting and translating clinical quality data to identify actionable items and develop best practices that improve clinical outcomes. The result is a more coordinated, efficient and qualitydriven approach to ambulatory healthcare.

St. Tammany Physicians Network (STPN) is a group of primary care physicians and practitioners employed by St. Tammany Parish Hospital. These clinicians address a full array of medical needs – from newborn care to geriatrics. STPN’s Quality Program began in 2014 with a single nurse, and by 2018 had expanded fourfold. The initiative provides STPN’s primary care providers with resources to identify patients with care opportunities. Adopting the Epic electronic medical records platform has been instrumental in achieving quality and population health goals. Utilizing Healthy Planet and other Epic capabilities has made a big impact on managing population health and increasing performance scores. The

Over time, the quality initiative’s impact has expanded into payor and value-based programs. Additional quality and population health-dedicated nurses were added to begin performing personaltouch outreach and better engage patients in their healthcare. Collectively, the impact of these qualityfocused measures has been meaningful across a wide range of metrics. Compared to baseline periods, colorectal and breast cancer screening increased 44% and 23%, respectively; adult pneumonia vaccination status increased 64%; blood pressure control advanced 8%; and Hemoglobin A1c control for diabetic patients improved 51%.

Ochsner and CHRISTUS Health: Delivering Diverse Benefits to Southwestern Louisiana OHN’s partnerships with other leading health systems in the region have been instrumental in our ability to deliver results for patients and all OHN stakeholders. Of these relationships is our collaboration with CHRISTUS Health Southwestern Louisiana. CHRISTUS has been an OHN partner since our founding in 2015. In 2018, that collaboration was strengthened through the creation of a joint venture between Ochsner and CHRISTUS designed to deliver a host of benefits to residents of Lake Charles and Southwestern Louisiana.

OPP Delivers Results for Daniel Clinic

The partnership encompasses two hospitals, a charitable foundation, an ambulatory surgery center, imaging centers and clinics. Expanding access and the availability of specialized care is a central goal of our alliance. According to Ernie Sadau, president and CEO of CHRISTUS Health, “One of the most beneficial aspects of the joint venture is our collective ability to recruit additional physicians, and to make a continued investment in critically needed service lines such as pediatric services, women’s services and cancer services, including expanded clinical trials.” Ochsner President and CEO Warner Thomas notes, “We’re excited to work with CHRISTUS to implement innovative technology that makes possible new, integrated care delivery options, while also enhancing population health. Jointly, we’re working to empower patients through our digital health offerings to effectively manage chronic disease.”

Since its founding nearly two decades ago, Daniel Clinic in St. Francisville, Louisiana, has evolved from a single-physician practice into a large clinic comprised of more than 30-person staff. More recently, its affiliation with OHN through participation in Ochsner Physician Partners (OPP) has been an important contributor to its success. OPP is a partnership between physicians, Ochsner hospitals and healthcare facilities within Ochsner Health Network that employs evidence-based methodology to improve quality performance and efficiency of care. Daniel Clinic founder, Dr. Chaillie Daniel, describes the benefits. “As we move into a quality-based world, OPP has helped identify weak points in our quality metrics and identify data that was not being conveyed. It’s kind of like adding light to a dark room. In the past, insurers collected the data and said, ‘Okay, here’s what is owed.’ OPP does a very good job of matching their knowledge and their data to be effective and to say, ‘This is our population, and this is what we’ve done for them’. It holds insurance companies accountable.” The premier benefit of the OPP relationship? “Clearly, it’s the opportunity for increased revenue,” says Dr. Daniel. “As an independent, two physician primary care practice, Daniel Clinic appreciates the impact of earning an extra $150,000 in valuebased payments this past year through their participation in OHN agreements.” Dr. Daniel adds, “OPP hasn’t altered our ability to choose how we interact with our patients, but their use of data to improve our practice and compete in a quality world has helped tremendously.”

THE OCHSNER ACCOUNTABLE CARE PLAN

Leading employers across the U.S. continue to explore new ways to provide high-quality healthcare and a better patient experience to their employees while, at the same time, reducing costs. It’s an objective that can be difficult to achieve through traditional delivery models. In 2018, OHN and Walmart came together with a common goal to create a new paradigm, one that benefits Walmart and Sam’s Club associates in the New Orleans Metro, North Shore and Baton Rouge communities. With the new Ochsner Accountable Care Plan (ACP), Walmart associates have access to a plan that simplifies, lowers or eliminates copays, coordinates care and provides access to thousands of providers – including 200 primary care physicians and 1,300 specialists who are part of OHN in the participating regions. The ACP encompasses Ochsner hospitals and clinics, along with St. Tammany Parish Hospital and Slidell Memorial Hospital, as well as their aligned community providers in the Ochsner Health Network. The ACP is available to Walmart associates at lower cost than a traditional preferred-provider organization plan. ACP participants have access to patient engagement specialists via a 24-hour call center designed specifically for their needs. It is an important plan feature that ensures Walmart associates are matched with the right care in the right place at the right time. Patients are directed to the most appropriate care within the network, and a coordinated complex case management program helps make sure the individualized needs of each patient are being met.

OHN AND BLUE CONNECT DELIVER FOR FIRST RESPONDERS AND THEIR FAMILIES

Effectively managing costs is fundamental to ensuring access to all who need care. In 2018, OHN announced a new partnership with the St. Tammany Parish Sheriff’s Office (STPSO) with the launch of the Blue Connect Network. Designed to reduce healthcare costs for employees and make dependent care more affordable, participants will enjoy reduced premiums, incur lower healthcare costs and find greater access to care.

This new plan reduces monthly premiums, affording more of our employees the ability to add dependents, and also allows the STPSO flexibility in paying a portion of dependent coverage.” Leaders f rom STPSO and four health systems worked with Blue Cross Blue Shield of Louisiana and HUB International to develop a program that would provide cost savings and high-quality healthcare. Such partnerships between employers and healthcare providers are playing an increasing role to make highquality care affordable and to help large and small organizations provide competitive benefits to their employees. STPSO is the first sheriff’s office in the state to offer this type of benefit to its employees. The Blue Connect partnership results in cost savings of approximately $500,000 annually for the STPSO.

The STPSO is the chief law enforcement agency in St. Tammany Parish and is comprised of more than 670 personnel across 34 departments. At OHN, we were excited to initiate a collaboration that delivers significant benefits to the women and men who protect the St. Tammany Parish community, as well as STPSO dependents. According to St. Tammany Parish Sheriff Randy Smith, “From day one, my goal has been to reduce health insurance costs for our employees and

The 2018 Summit brought together healthcare industry decision-makers from across the nation to explore emerging approaches and to share best practices for delivering high-quality care in today’s value-based environment.

Thought leader presenters included Adam Boehler, Director of the Center for Medicare and Medicaid Innovation; Thomas Lee, MD, Chief Medical Officer of Press Ganey; and Dennis Weaver, Chief Clinical Officer of Oscar Health. The event drew administrative and clinical healthcare leaders from health systems, accountable care organizations, hospitals, group practices, payor organizations and life sciences companies from across the United States. Quality Blue Primary Care

Coupled with a 20% increase in overall population, OHN’s Quality Blue Primary Care performance in 2018 saw great improvements in outcomes in each of the high-priority chronic disease initiatives. Not only does this mean that patients served by OHN physicians had their conditions better managed and their health status improve in 2018, but our practices were rewarded with 50% higher care management fees – or bonus payments – for the betterthan-average outcomes. Additionally, 147 OHN PCPs were recognized as top performers in the care of one or more chronic diseases, representing nearly 40% of all PCPs recognized as High Performers in the state of Louisiana.

UnitedHealthcare ACO

In 2018, OHN’s UnitedHealthcare ACO population grew 33% across this statewide accountable care program. Even with this tremendous growth, OHN surpassed the quality program target in its UnitedHealthcare ACO, exceeding the National Quality Compass 90th percentile in breast cancer screening rates, with a network-wide compliance rate of over 81%. With a focus on cancer screening compliance, OHN also saw improvements in cervical cancer screening and colorectal cancer screening rates to 76% and 66%, respectively.

Internal Performance Measurement

A key strategic initiative in 2018 was the creation of a measurement tool so that the network would not be reliant on health plans to report outcomes several months in arrears. Leveraging a variety of tools and advanced technology, including Epic, IBM Watson Health and Tableau, OHN built the ability to track its performance on over 40 measures, using both industry standard measure specifications and internally created algorithms. In addition to developing, testing and deploying the measures, accurate monitoring required the acquisition and integration of clinical and claims data from a variety of provider and payor data sources.

SOLUTIONS AND INITIATIVES Leadership, Partner and Physician Education We conduct educational programs for boards, committees and CINs to help them strengthen their capabilities and familiarize them with emerging trends and innovative approaches. In addition to presentations and content at the 2018 Value Summit, educational topics included OHN programs designed to help physicians track the latest developments relevant to the success of their practices. OHN’s Performance Improvement team works with each partner to review processes, identify opportunities for improvement and deploy initiatives that support continuous improvement within the framework of payor-defined quality and cost metrics. We implement standardized reporting across diverse areas focused on a distinct group of performance measures. In addition to three imperative metrics, each OHN partner identifies five additional utilization or quality measures as areas of emphasis for their performance improvement activities.

Evaluating New Directions: In a population-health based environment that drives utilization down, OHN could deliver more value to providers by gaining access to premium and offering insurancetype products. Today we’re evaluating potential changes to our model by creating a single unified network that would do more to capture the benefits from the improved outcomes we’re delivering.

Our Going Forward Goal: OHN is committed to doing even more to provide the expertise, capabilities and support infrastructure providers need in order to flourish in value-based healthcare delivery. The Challenge: Driven by CMS, Medicaid and by commercial payers, providers are being pushed away from fee-for-service, volume-driven healthcare toward proactive, population health-based, valuebased care delivery and payment. While that is a trend with many benefits for patients and other stakeholders, it also creates financial challenges for independent providers as they seek to compete in this new healthcare paradigm.

How OHN Can Add More Value: We are working to collaborate more closely, and to become even more tightly aligned, with our provider partners to support their transformational journey. We’ll achieve this through the deployment of improved analytics and digital tools and capabilities such as call centers, care coordinators and disease management programs – broadening the population of physicians with access to these tools. We’re also forging deeper integrated partnerships with the major payers in our state: Blue Cross, Humana and United.
