The aim of the present survey was to analyse the frequency and type of antibiotic prescriptions used in dental practice in Saudi Arabia, to assess the awareness of periodontal treatment need among dentists and specialists and to evaluate the distribution of dental specialties in Saudi Arabia. We distributed a questionnaire to 378 dentists including different specialists working in various major hospitals and dental departments. This same questionnaire was then redistributed, 3 years later, to 211 participants. Results from both surveys indicated that antibiotics were not always prescribed on a scientific basis. Cost was the most significant factor (p < 0.05). Six major bands corresponding to penicillin-binding proteins (PBPs) with molecular weights ranging from 43,000 to 97,000 were detected in cell envelopes of Yersinia pestis EV76 grown at 28 degrees C. When cells were transferred to 37 degrees C and incubated for extended periods of time, the amounts of all PBPs, except for PBP2, were gradually reduced in cell envelopes of a strain carrying a 75-kb virulence-associated plasmid (as measured by penicillin-binding capacity), whereas in a strain cured of the plasmid, all PBPs were stable. The results indicated that the stability and/or the expression of Y. pestis PBPs is affected by a temperature-inducible pathway associated with the virulence-associated plasmid.

Transformation of a type I SCC mec element into Staphylococcus aureus yielded highly oxacillin-resistant transformants with a reduced growth rate. Faster-growing variants could again be selected at the cost of reduced resistance levels, demonstrating an inverse correlation between oxacillin resistance levels and growth rate.

A nationwide multicenter susceptibility surveillance study (Susceptibility to the Antimicrobials Used in the Community in España [SAUCE] project), SAUCE-4, including 2,559 Streptococcus pneumoniae , 2,287 Streptococcus pyogenes , and 2,736 Haemophilus influenzae isolates was carried out from May 2006 to June 2007 in 34 Spanish hospitals. Then, the results from SAUCE-4 were compared to those from all three previous SAUCE studies carried out in 1996-1997, 1998-1999, and 2001-2002 to assess the temporal trends in resistance and the phenotypes of resistance over the 11-year period. In SAUCE-4, on the basis of the CLSI breakpoints, penicillin (parenteral, nonmeningitis breakpoint) and cefotaxime were the antimicrobials that were the most active against S. pneumoniae (99.8% and 99.6%, respectively). The levels of immune complex-like material in serum from 18 patients in connection with skin eruptions appearing during penicillin therapy were determined with a modified ELISA. Immune complex-like material could only be shown in the serum of 3 out of the 17 patients. However, according to ELISA results, the complexes appeared without the simultaneous presence of penicilloyl-specific antibodies. Neither could penicilloyl-specific antibodies be recorded in 9 patient treated with parenteral penicillin without adverse reactions. 3 of these patients had immune complex-like material in their serum. It was concluded that immune complex-like material with penicilloyl specificity is not likely to contribute to the development of skin eruptions often seen in connection with penicillin treatment.

ABSTRACTThis randomized, double-blind, multicenter trial compared the efficacy and safety of linezolid, an oxazolidinone, with those of oxacillin-dicloxacillin in patients with complicated skin and soft tissue infections. Syphilis has become less common in Europe in the last decade, but has once again become a major problem in the USA, and remains so in many developing countries. Several treponemal genes have now been cloned and expressed in Escherichia coli, allowing study of treponemal proteins. The importance of cell mediated immunity in syphilis has been demonstrated in animal models. A diagnosis of syphilis is usually confirmed by dark-field microscopy or serological tests. Seroconversion may be delayed in HIV infected individuals. A positive reaginic test in cerebrospinal fluid (CSF) has a high specificity but low sensitivity in the diagnosis of neurosyphilis. Indeed, virulent treponemes can be identified in CSF samples which have negative reaginic tests, normal cell counts and protein levels.
