Sheldon Marks, MD responded:
They often hang at different levels. As far as size, the left is often smaller because of a cluster of veins above the left testicle that keep the testicle from growing to its normal size. Because testicles are in the scrotum to keep them several degrees cooler than core body temperature, the extra veins bring extra heat and this may be the cause. See a urologist and get checked out as this may need to be fixed. If significant, a varicocele can hurt your future fertility and even testosterone production. The sooner you get it fixed the better on many levels. I prefer a microsurgical repair.

I'm 52 and have a similar anatomical situation. I've fatherd 5 children and no woman has ever complained. This is normal.

Totallly agree that this is not a serious situation. Don't let some knife-happy surgeon talk you into some unnecessary procedure.

You might also ask a urologist whether you have a hydrocele in the larger testicle. A hydrocele is a sac of water in or on the testicle that causes the entire structure to enlarge. It's not life-threatening, and it can be repaired. It's thought to be congenital, and is not related to self-abuse. But unless the repair is done correctly the first time, it can mean further procedures in the future, one of which may be a vasectomy that you might not want, especially if you're young and would like to father children later.

is it normal that both testicles are almost never in the scrotum?

Sheldon Marks, MD replied to MaximusVictorious's response:
No. they should be down- best time to check is in a hot shower. Any questions see a urologist.

Yesterday my left teste, with varicocele hung at least 4-6 inches longer than right side. First time that I noticed such a major difference. Had been on androgel but stopped because endo said more than a bllod test needed to be done, Internist not happy that I'm on "T" at my age(75) 1st blood test was 160 last (before stopping) 51. Having urol exam this week and MRI of head (a second one) Lo "T" leaves me vey tired and weak.

Torcal replied to Sheldon Marks, MD's response:
This is probably just a nit pick but it has always been my understanding that the temperature of the testes is regulated by the Scrotum which distends when the body is warm and retracts when the temperature becomes too low. My left testicle is slightly larger than my right one and I can't feel any cluster of veins that restricts it. And the right one hangs down lower than the left. I have always assumed that the differing levels prevent the testes from slamming against each other while running away from an angry Wildabeast that didn't die after being hit by the first spear.

Having closely examined numerous sets of testes, I can attest to a wide range of "normal". From motility (yes, movement!), size differentials, cold-responses, swelling during arousal, scrotal variations, bags of balls are like snowflakes! Generally though, one is slightly larger, one is slightly higher.
Small testicles can produce large amounts of ejaculate just as large ones can be mostly decorative...youthful angst over "normality" is generally unfounded, thankfully. But as with breasts, self-examination is a useful tool. Pain, changes in texture, a new addition to your usual "set" should send you to your primary care physician.
There are ways to damage your nuts but thankfully they have been programmed by natural selection to "walk it off". Just be careful- they are kind of out there on their own...

Yeah, I remember a semi-humorus joke about life lessons drawn from baseball. Of all the different things that you can learn from playing the game over the years perhaps the most important one is this: always wear your cup!!

I've also read that through evolution/selective breeding that because a male's testicles are different sizes and hang at different levels we were able to walk upright more easily (less banging into each other and less pain) and therefore could better protect and procreate. Anyone else heard this explanation? And yes I agree always wear a cup! better to be safe than sorry! And back to the orignal post, if there is significant difference in sizes there may be a more serious issue at hand. Swallow your pride and have a medical professional check into the cause.

I have varicocele for many years it didn't hurt before until a few days ago it started hurting. What should I do?

Sheldon Marks, MD replied to Torcal's response:
Excellent points and I like the logic to explain why they do hang slightly different levels. I think I may use that in future speeches.
