If you test positive for COVID-19 you can share your information with us securely online. If you test positive and have a valid mobile phone number, you will receive a link and access code to complete this form.
What the form is for
The contact tracing form lets you share:
- any symptoms or underlying health conditions you may have
- details of your household contacts
- details of any high-risk locations.
It also includes instructions on how to share your NZ Covid Tracer Bluetooth data.
At the end of the form you can apply for welfare assistance with the Ministry of Social Development.
Completing the form
Completing the form online has been made simpler. For most people, filling out this form may take 5 to 10 minutes to complete. Once you have received the text message, you can submit the form anytime within 72 hours. If you do not submit the form within 24 hours, someone may call you and complete the case interview over the phone.
