Some certificates are now available through e-mail. If the certificate
you are requesting is available electronically, the e-mail column on this request will be enabled
with a check box. If you check this box, the certificate will be forwarded to the e-mail address entered
on this request. If you do not check the box, the certificate will be delivered through regular mail.
