Fill out a downloadable form and mail it in. All orders must be prepaid by check or money order payable to the Commonwealth of Massachusetts. Please allow approximately one week for delivery. Prices are subject to change. We require a street address and phone number to ensure proper delivery. Orders should be sent to the following address:
