Downloadable Forms

Downloaded forms are currently available for corporations, trademarks and service marks. Limited liability companies and partnerships must follow the guidelines provided and draft their own documents. Downloaded forms must be printed, completed, signed with an original signature and submitted by either mail, fax filing, or walk-in. To download the forms, you must have the Adobe Reader installed on your computer.

Please note: All forms must be printed on 20 lb. paper to comply with filming requirements

Fillable Forms

Fillable forms are currently available for domestic profit corporations, foreign corporations, and professional corporations only because the filings for these entities must be typed. The fillable forms can be opened and downloaded as provided in the section above. Follow the instructions for obtaining Adobe Reader, if necessary. Simply click on the forms in the desired fields, type in your information and print out the document with all your information included. Fillable forms must be typed on your computer, printed, signed with an original signature and submitted by either mail, fax filing, or walk-in. To download the forms, you must have the Adobe Reader installed on your computer.

UCC

We do not supply Massachusetts U.C.C. Forms to the public. They may be purchased at most legal stationers throughout the state. All U.C.C. filings must be typed. Our office will accept any state's forms provided that the entire form is presented intact. Please do not remove carbons and do not interlace or staple oversize documents or attachments to the forms. Please do not write anything in the box marked "For filing officer use." Failure to comply with the above policies may result in delayed processing of your U.C.C. document.
