Deadlines for Submission

The trainee is responsible for submitting the completed online form to the relevant State, Territory or New Zealand Office.

• The Three-monthly Formative Appraisal (Three-monthly report) is due within four weeks of completion of the three-monthly training period  
• The Six-monthly Summative Assessment (Six-monthly report) is due within six weeks of the completion of the six-month training period.

2021

These dates apply to trainees in both Australia and New Zealand:
