America’s Community Health Centers collectively serve more than 27 million people in more than 9,800 rural and urban communities across the country. For more than 50 years, their community-directed and patient responsive model of care has provided accessible, quality health care to medically disadvantaged and underserved people – regardless of their ability to pay. Today the health center model plays a critical role in expanding health care to more people and in transforming the nation’s health care delivery system by lowering health care costs, reducing chronic disease and health disparities.
