2013 Funding (Year 4)

  • Amount of funding: $579,977
  • Description of Project
    Awardee will:
    • Finalize the community health assessment, develop a community health improvement plan, and begin drafting a department strategic plan
    • Create a workforce development plan based on the nationally adopted public health core competencies
    • Create performance measures and performance reports for program divisions and continue performance reporting and review activities with the administrative divisions
    • Achieve performance improvement in the Office of Food Protection and in the Medical Examiner’s Office
    • Continue to build and deepen cross-jurisdictional relationships that integrate public health and health care
    • Tap additional data sources related to hypertension, adult immunization, and cancer screening
    • Expand the amount of adult immunization data in the Philadelphia immunization registry

2012 Funding (Year 3)

  • Amount of funding: $664,213
  • Description of Project
    Awardee will:
    • Complete the community health assessment, strategic plan, and accreditation self-assessment
    • Develop each program’s key services, mission, and performance measures
    • Identify performance improvement projects that will streamline processes and increase efficiency
    • Work with insurers, providers, and community-based agencies to expand data access and analytical capacity

2011 Funding (Year 2)

  • Amount of funding: $664,213
  • Description of Project
    Awardee will:
    • Model a process of documenting, collecting, and reporting performance in human resources and automate the hiring processes to fill vacancies in a more timely manner
    • Demonstrate that routine review and discussion of performance data can improve results through the use of process mapping, enhancements in data collection, and the use of data in decision-making
    • Introduce a training module to expose all functional units to the concepts and objectives of performance management
    • Develop improved collaboration and better integration of personal health care and public health services
    • Promote population health by creating the infrastructure to link personal and public health services to drive better health outcomes
    • Maximize the public's health status by strengthening the Philadelphia Department of Public Health's ability to access, integrate, and use health care system data through monitoring and intervening in the promotion of health care access and quality, promoting communication and interaction between health care providers and systems for health promotion, and informing the public of changes in programs and systems that will promote their health

2010 Funding (Year 1)

Component I

  • Amount of funding: $100,000
  • Description of Project:
    Awardee will:
    • Focus on building, institutionalizing, and implementing performance management capacity within health departments
    • Support full-time "Performance Improvement Manager" who will participate in a national network of performance improvement professionals supported by CDC

Component II

  • Amount of funding: $1,018,493
  • Description of Project:
    Awardee will enhance public health improvement efforts beyond Component I through two or more categories including: performance management, policy and workforce development, public health system development/redevelopment and best practice implementation.
