Government Visa Travel

CDC employees please visit the Global Travel Office site for a listing of visa requirements for official U.S. passports for entry into foreign countries. The listing shows the number of visa application forms with original signature and photos required to apply for the Visa as well as any other requirements (i.e. letter of invitation, OMEGA printed itinerary, etc.). 

Please contact the CDC Passport/Visa Services Office (404-718-8890 or email CDCPASS@cdc.gov) for assistance in completing your application package. This statement is located in the first paragraph near the end. The Request for Passport/Visa Services (CDC form 0.1308) is required for all passport and visa requests.

It is important to note that entry requirements for U.S. citizens traveling on official U.S. passports often vary from entry requirements for U.S. tourists. Entry requirements for foreign passport holders also vary and entry requirements must be verified with the CDC Center for Global Health (CGH) on a trip-by-trip basis.

NOTE: It is the traveler's responsibility to obtain letters of invitation, visa receipt numbers (Russia) and visa authorization issuance numbers (Vietnam).
