The Opportunity:

Vermont CARES is looking for two full-time Medical Case Managers: one position primarily serving the Montpelier area, and one position primarily serving the Burlington area, with travel between these and other regional offices. These people will make a huge difference in the lives of our HIV+ neighbors and be part of a supportive team.

The Organization:

Vermont C.A.R.E.S. (the Vermont Committee for AIDS Resources, Education, and Services) provides support services to about 170 Vermonters living with HIV/AIDS and their family members; reaches over 5,000 Vermonters each year with HIV prevention education, skills-building and materials; and administers free, anonymous, rapid-result HIV tests. The main office of Vermont CARES is in Burlington with regional offices in Montpelier, St. Johnsbury and Rutland. See: www.vtcares.org 

The Position:

Medical Case Manager is a dynamic position working with HIV-positive individuals, their families, and their communities. Prevention activities will also be a large part of this position and will include HIV testing, educational presentations, and outreach throughout the state. This full-time position (37.5 hours per week) is based in Burlington and/or Montpelier and offers generous benefits: health, vision, dental, and ample paid time off.  Starting salary: $28,000-$30,000. Knowledge of HIV/AIDS, community resources, and the harm-reduction model is necessary. Reliable transportation needed.
