Treatment 2015 provides a results-driven framework to expedite and greatly expand coverage. With less than 1000 days before the end of 2015, much work remains to be done. The WHO’s new 2013 guidelines on The Use of Antiretroviral Drugs for Treating and Preventing HIV Infection recommend a CD4 threshold of 500 for initiation of HIV treatment. As an important step towards getting to zero AIDS-related deaths, countries should be encouraged to prioritize immediate efforts to ensure that all people eligible for HIV treatment have access to it.

We are encouraged by the progress made by countries in reducing the number of children newly infected with HIV. Two years have passed since the launch of the Global Plan, when countries embraced the goal of moving towards eliminating new HIV infections among children and keeping their mothers alive. Since then, there has been momentum in scaling up access to HIV prevention and treatment services for women and children – especially in the 21 Global Plan priority countries in sub-Saharan Africa. Ghana and South Africa have led the way, with the largest decreases in the numbers of new HIV infections among children from 2009 to 2012.

The African Union’s 50th anniversary provides a unique opportunity to reflect critically on African efforts and progress on its developmental path but also on its evolving relationships with the external partners who have joined the Continent on this journey. It is particularly opportune to do so in the context of unprecedented shifts in poles of geo-political power and new approaches to development cooperation and finance.

As the world economic landscape changes, so too does the HIV funding landscape. The limited resources available require more emphasis on value for money. This case study report consists of eight case studies. It highlights countries’ progress in making their HIV response more efficient or increasing domestic HIV funding, contributing to sustainability, increased scale-up and country ownership. Cambodia and Myanmar have re-allocated resources towards high-impact interventions. South Africa and Swaziland have saved millions by improving their antiretroviral drug tenders. Kenya, Namibia, Malawi and Kazakhstan have taken active steps for a future with fewer external funds. Each country has evolved strategies that other countries may apply to their particular context. The examples given here aim to catalyse country-driven action to make efficiency and sustainably funded HIV services the reality in the HIV response.
