Interpreting Abnormal Pap Tests

Because a Pap test screens for abnormal cervical cells and because those cell changes can be associated with cervical cancer, being on the receiving end of an abnormal Pap test result can be frightening, intimidating, and confusing.

On the “frightening” aspect: Some people assume that an abnormal Pap means that cervical cancer is imminent. On the contrary, the National Cancer Institute not only states that cervical cancer, when it develops, takes “many years” to do so, but also that “[t]he majority of infections with high-risk HPVs [human papillomaviruses] clear up on their own.” This is not to suggest that regular screenings aren’t important — but rather, that they do their job and detect cervical changes in plenty of time to prevent cancer from developing in the first place.


Remember that if you’re confused about your Pap test results, part of your health care provider’s job is to answer your questions and keep you informed.


On the “confusing”: It’s true. There are a lot of different letter codes. Though some of them look awfully similar, they each mean a different thing. Moreover, the clinical recommendations for how to follow up with an abnormal Pap can depend not only on the code — in other words, the specific abnormal result — but also on one’s age and medical history.

STD Awareness: “What Are the Symptoms of an STD?”

“I was treated for chlamydia, but my girlfriend feels fine, so she doesn’t need to get tested.”

“The only time I don’t use condoms is for oral sex, and everything’s been OK ‘down there,’ so getting tested for sexually transmitted diseases would be pointless.”

It’s important to be able to recognize the symptoms of a sexually transmitted disease (STD). Being savvy about symptoms can push you to get tested right away if you notice that something is amiss. However, being symptom-free can lull you into a false sense of security, especially if you’ve engaged in sexual activities that could have exposed you to an infectious agent. The fact of the matter is that many people with STDs have no symptoms at all. As they say, “The most common symptom of an STD is … no symptom.” Let’s take a look at some common STDs.

Bacterial Infections

Bacterial STDs are curable with antibiotics. They include chlamydia, gonorrhea, and syphilis — all of which can be asymptomatic, and all of which can have severe complications when they are not treated in time.

Chlamydia: Around 3 million Americans are infected with chlamydia annually, and the infection is especially common among young people (less than 25 years of age). Chlamydia can infect the penis, vagina, cervix, anus, urethra, eye, or throat. You can be afflicted with a range of symptoms: pain or a burning feeling while urinating; vaginal, cervical, or penile discharge; swelling around the anus, testicles, or vagina; and more.

However, you’re much more likely not to experience any symptoms at all — most people with chlamydia are unaware they have it. Three out of four women with chlamydia have no symptoms, and half of men with chlamydia have no symptoms. Left untreated, chlamydia can become a serious health threat. Long-term complications might lead to fertility problems and arthritis. Continue reading

How Often Do I Need a Pap Test?

Almost 80 years ago, Dr. George Papanicolaou developed a simple test, the Pap test (also called the Pap smear), done in a doctor’s office to check for cervical cancer. During a pelvic exam, a doctor swabs a small sample of cervical tissue and looks for abnormal cells. If these precancerous cells are detected, it will lead to more tests or other more invasive treatments such as a colposcopy (in which actual tissue may be removed). In the 1930s, when Papanicolaou was developing his test, cervical cancer was more lethal than breast cancer. But since the development of this test, the number of women dying from cervical cancer has dropped dramatically. In 2009, of the 4,000 women in the United States who died of cervical cancer, most had never been screened or had not been screened in the 10 years before their diagnosis.


This year, the U.S. Preventive Services Task Force recommended less frequent Pap testing.


Cervical cancer is most common in women between ages 35 and 55, and usually develops from a human papillomavirus or HPV infection. Not all HPV infections lead to cervical cancer, and it can take decades for a persistent infection with a high-risk type of HPV to become cancer. High-risk HPV types are sexually transmitted and can lead to cervical cancer and also anal, penile, and oral cancers.

There are two types of screening: Pap tests and HPV tests. While they both require a pelvic exam in which cells are taken from the cervix, Pap tests look for abnormal or precancerous cells, and HPV tests look for DNA or RNA from high-risk HPV types in cervical cells. Both tests are used to try to catch cervical cancer in its earliest stages so that it can be successfully treated. Continue reading

Pap tests are not beloved — there, I said it. Many people who receive them hope for the day that medical science devises an equally effective but noninvasive test, whatever that might entail, as they may find them to be anything from slightly awkward to incredibly traumatizing. However, the evidence overwhelmingly shows that access to Pap testing (also called Pap smears) has transformed cervical cancer from a top killer into something that can be caught early and treated before it can do much damage. As a tool to reduce cancer deaths, Pap tests have been a resounding success.


In countries with widespread access to Pap tests, cervical cancer rates have plummeted.


A Pap test involves an instrument called a speculum being placed into the vagina to hold it open while a health care provider uses a small spatula or a tiny brush to collect cells from the cervix, which is at the opening of the uterus. These cells are analyzed in a laboratory, where a technician can determine if they are precancerous. (Cervical cancer is caused by a sexually transmitted virus called human papillomavirus, or HPV.) When caught in its precancerous stages, cervical cancer can be treated with procedures such as colposcopy and cryotherapy.

Welcome to the latest installment of “Over 90 Percent of What Planned Parenthood Does,” a series on Planned Parenthood Advocates of Arizona’s blog that highlights Planned Parenthood’s diverse array of services — the ones Jon Kyl doesn’t know about.

You know what they say: An ounce of prevention is worth a pound of cure. Public health experts agree that vaccines are one of the most important advancements in medicine, and are incredibly safe and effective in preventing infectious disease. Many infectious diseases that used to lay waste to their victims are now unknown to many of us in the developed world — polio, whooping cough, measles, and rabies struck fear in the hearts of our forebears, but most young people today barely know what they are (although low rates of vaccination can still lead to outbreaks, such as 2008’s measles outbreak in Tucson). Smallpox, once a terrifying scourge, has been wiped off the planet thanks to vaccination campaigns.


We offer vaccinations against hepatitis A and B, as well as HPV.


Vaccines work by introducing antigens to your immune system. An antigen is a substance, such as a protein on the surface of a virus, that the immune system can recognize as dangerous. It is then able to attack the pathogen and, often, create a “memory” of that pathogen so it can attack it in the event of reinfection. The antigens in vaccines are very safe, and can be derived from many sources, such as inactivated (dead) or attenuated (weakened) pathogens, or fragments of pathogens. Some vaccines, such as those protecting against human papillomavirus and hepatitis B, are made with laboratory-synthesized fragments of the surface proteins of viruses, which are sufficient to produce immune response despite being completely noninfectious.

Planned Parenthood Arizona offers vaccinations against hepatitis A, hepatitis B, and human papillomavirus (HPV). All three of these viruses can be transmitted sexually, and hepatitis B and HPV can cause cancer if the infections become chronic. The hepatitis vaccines have led to all-time lows in rates of hepatitis A and hepatitis B; the HPV vaccine is still new, but emerging evidence suggests a possible decrease in HPV rates as herd immunity grows. STD Awareness: HPV in Males

Human papillomavirus, or HPV, affects people of all genders.

Whether it’s worries over Gardasil making girls go wild, or it’s somber discussion about cervical cancer, discourse about human papillomavirus (HPV) centers around its impact on females. But who are most of these females getting HPV from? For the most part, they’re getting it from male partners. And despite the fact that cervical cancer is the most common cancer associated with HPV, it is not the only one. A high-risk strain of HPV can lead to cancers of the penis, anus, mouth, and throat; additionally, there are strains of HPV that cause genital warts, which affect males and females equally. So why don’t males figure very prominently in discussions of HPV and the preventive vaccine, Gardasil?


Mouth, throat, penile, and anal cancers can all be caused by HPV.


Some people think that if they remain abstinent until marriage, they will be able to avoid sexually transmitted diseases (STDs) – but not very many people can say with certainty that their spouses have never had any other sexual partners. Eva Perón, the second wife of Argentine president Juan Perón and a leader in her own right, was made famous here by the musical Evita. According to physician and writer Shobha S. Krishnan, she died in 1952 of cervical cancer – the same fate that befell her husband’s first wife. Many believe that Juan Perón was the source of both women’s ultimately fatal HPV infections.

While one’s own sexual behavior can increase risk for acquiring an STD, it is not the only factor – the sexual history of one’s partner also plays an important role. HPV is especially tricky because there is currently no FDA-approved test for HPV in males – despite the fact that more than half of sexually active males are estimated to have been infected with HPV at some point in their lives. And, because it is so often asymptomatic, a male can carry this virus without knowing it, unwittingly infecting his partners. Continue reading

The HPV Vaccine Controversy: Sex, Cancer, God, and Politics: A Guide for Parents, Women, Men, and Teenagers

The HPV vaccine, released in 2006, was ripe for controversy, at least in places like the United States. Here there is a strong anti-sex undercurrent from certain segments of society, and fears abound that a vaccine that protects against a common sexually transmitted disease — especially one whose symptoms disproportionately affect females — would encourage sexual promiscuity among our nation’s teenage girls. In addition, there is a segment of society that is deeply suspicious toward vaccines, a fear that is often fueled by misinformation or misunderstanding.


The HPV Vaccine Controversy is an excellent resource for anyone considering vaccination, as well as those who have already been exposed to human papillomavirus.


While Krishnan’s book is an invaluable guide for anyone considering the vaccine for themselves or their child, it covers much wider territory than just the vaccine and its attendant controversies. The first half of the book is devoted not to a discussion of vaccination but to a thorough and accessible description of female anatomy (although apparently her claim about the teenage cervix is controversial), the lifecycle and transmission of human papillomavirus (HPV), cancer screening techniques such as the Pap test, and the slow development of cancer caused by HPV infection. It also has good information on genital warts, which are caused by certain strains of HPV (such as HPV-6 and HPV-11) that often get overlooked in discussions of their cancer-causing cousins (such as HPV-16 and HPV-18). This makes the book an excellent resource for anyone who has had an abnormal Pap test and has questions — the detailed descriptions of the various cervical-cell abnormalities and the different stages of cervical cancer will assist the lay reader in making sense of her diagnosis.
