Procedures for Pre-Season Physicals

1st Year Athletes and Transfers (High Risk Sports)
 
1) All first year athletes who are on the six high risk teams, (Football. M&W Rugby, M&W Hockey, and Wrestling) will complete the online E-ppe forms (university.e-ppe.com) no www.   Once completed print off the entire form, and bring it to their team’s first meeting or practice and hand it in to the teams assigned student trainer. 
2) ** please note if a 1st year athlete in this category knows that they cannot make their assigned physical date they can have the form completed by their family physician.
3) *** NO ATHLETE WILL BE ALLOWED TO PRACTICE UNLESS THEY HAVE COMPLETED THE PRE- SEASON E-ppe FORM AND HAS TURNED IT IN. NO EXCEPTIONS!!!!!
4) Once completed, the forms will be reviewed, and returned to you at your assigned team medical clearance day at the Fowler/Kennedy Sport medicine clinic. 


1st Year Athletes and Transfers (Non High Risk)
 
1) All first year athletes who are not on the six high risk teams, (Football. M&W Rugby, M&W Hockey, and Wrestling) will complete the online E-ppe forms. (university.e-ppe.com) no www. Once completed, you will print off the medical history and assessment form and take it to your family doctor or an outside physician to be completed.
Once you have your physical E-ppe form completed you will bring it with the physicians clearance form to your first tryout practice and or team meeting and give it to the team head coach, or student trainer assigned to your team.
2) *** NO ATHLETE WILL BE ABLE TO PRACTICE UNLESS THEY HAVE COMPLETED THE PRE- SEASON E-ppe FORM WITH A PHYSICIANS CLEARENCE AND HAVE TURNED IT IN. NO EXCEPTIONS!!!!!!
3) Once the forms are collected they will be given to Rob Walsh the Coordinator of Athletic Therapy services for review.

Procedures for Pre-Season Screening

Returning Athletes (High Risk Sports)
 
1) All returning athletes in the six high risk sports ( M&W hockey, M&W Rugby, Football, and Wrestling) must fill out the Returning Athlete Update Form, found on the SRS / Athletics website
2) Print off the PDF form. Once you have filled out the form be sure to bring it with you to your teams 1st practice and or meeting.
3) NO RETURNING ATHLETE WILL BE ALLOWED TO PRACTICE UNLESS THEY HAVE COMPLETED THE UPDATE FORM AND HANDED IT IN TO THERE COACH OR STUDENT TRAINER.
4) THESE UP DATE FORMS WILL BE REVIEWED By Rob Walsh (coordinator of Athletic Therapy Services) AND THOSE NEEDING A CONSULTATION WITH A PHYSICIAN WILL BE NOTIFIED ON THEIR TEAMS MEDICAL CLEARENCE DAY, NO EXCEPTIONS!!!! 
 
*****Please note all returning athletes must still attend their teams designated medical clearance day regardless if they need as physicians appointment. While attending the Medical Assessment Day these athletes must still complete a baseline concussion assessment for this current year.

Returning Athletes (non High Risk Sports)
 
1) Returning athletes. All returning athletes excluding the six high risk sports (Football, M&W hockey, M&W rugby, and Wrestling) must fill out the returning athlete update form. This form is available as a printable PDF form and can be found on the SRS / Athletics website.
2) Print off the form. Once filled out, all returning athletes must bring this completed update form to their teams 1st practice and or meeting and hand it in to their head coach or student trainer if applicable.
NO RETURNING ATHLETE WILL BE ALLOWED TO PARTICIPATE IN ANY PRACTICES UNLESS THEY HAVE COMPLETED THE UPDATE FORM AND HAVE SUBMITTED IT TO THE PROPER PERSON. NO EXCEPTIONS!!!!!
 
****Please note: All forms will be reviewed by Rob Walsh (Coordinator of Athletic Therapy Services) if it is felt that further medical follow-up is necessary you will be contacted with regard to making an appointment to see one of the physicians at The Fowler / Kennedy Sport Medicine Clinic.
Please remember that any athlete can book an appointment with any physician at the Fowler / Kennedy Athletic Clinic at any time concerning any issue they might have.

CONCUSSION INFORMATION
Due to the recent breakthroughs in concussion identification and management it is being recommended by many experts in the field, that all athletes from every sport, at all levels of participation get a base line concussion test performed. Presently at Western only those athletes in the identified six high risk sports (Football, M&W Rugby, M&W hockey, and Wrestling) get these done. It is being recommended that all of our varsity athletes get this baseline testing done, as a preventative measure prior to their participation in their sport. If you have already done a computerized baseline test in the past (e.g. Impact) then there is no need for you to redo this test.
This is just a recommendation and is not mandatory. If you wish to perform a baseline concussion test on yourself as a preventative measure, that will be kept on record please go to the following web site and follow the instructions there. www.impacttestcanada.ca
