REGISTRATION GUIDELINES
1. A parent or guardian must complete the
Online Registration or submit a completed
Registration Form.
2. Please create one account / use one registration form per child.
3. Please include information regarding allergies, medical concerns, and special needs.
4. A parent or guardian must read and SIGN the indemnification wavier (on the registration form or online).
5. Payment must accompany the registration (no registrations will be processed without full payment).
6. Cheques should be made payable to
The University of Western Ontario ~ Sport Western.
7. No post-dated cheques will be accepted.
8. Please do not send cash in the mail.
9. A $25.00 service charge will be issued for an NSF (Non-Sufficient-Funds) cheque.
10. All prices include HST if applicable.
REGISTRATION OPTIONS
ONLINE
Online Registration opens on February 7, 2011 at 8AM.
STEP 1:
Sign In or
Sign Up (Note: Your Summer 2010 camp registration login information is no longer valid.)
STEP 2: Add all the children (dependents) to your account that you wish to register for camps.
STEP 3: Search for camps via the
Courses menu option, add desired camps to your
cart, and follow prompts/complete all required information fields.

ensure that you are searching for camps under the "SUMMER 2011" section.
STEP 4: Proceed to Checkout (complete credit card and billing information. Print confirmation page for your records.)
IN-PERSON
Register at
Campus Recreation - Membership Services seven days a week. Membership Services is located in the
Western Student Recreation Centre. For hours of operation please
click here. We accept cash, cheque, MasterCard, VISA, and Direct Debit.
MAIL
Complete the
Registration Form and mail with full payment to:

* A confirmation receipt will be E-Mailed to you once your registration has been processed. Your confirmation E-Mail is your tax receipt; keep it in a safe place.
REFUND (CANCELLATION) POLICY
All cancellations are subject to the following conditions:
1. All requests for a refund must be sent via E-mail to
sportwestern@uwo.ca
2. 25% non-refundable administration fee is applied to any and all cancellations.
3. No refunds will be given with less than 14 days notice before the start date of the camp.
4. No Show = No Refund.
5. In all circumstances, a full refund will be issued for withdrawals due to medical reasons, with a medical certificate.
