2011-12 e-PPE LINK AND INSTRUCTIONS

Western 2011-12

Western now uses an electronic Health History Evaluation (e-PPE) questionnaire prior to your actual pre- participation physical to assess health and injury history prior to participation in sport. This document is to help orient you to the questionnaire.

 

Overview

Identifying risk factors that predispose an athlete to injuries or illnesses is the fundamental purpose of the

e-PPE.  Medical experts are convinced that a detailed, comprehensive and validated PPE can assist in over all management of the athlete discovering previously undetected potential medical problems that pose a risk to the competitive athlete.

 

How does the e-PPE work?

The e-PPE is an internet based, comprehensive and detailed risk-assessment questionnaire that each student athlete completes. All UWO Athletes must complete this questionnaire. If the Athlete has any concerns our Sport Medicine Physicians are more than happy to help.

Once there  - click - on - University of Western Ontario – you will be asked to confirm your choice - this will take you to the registration page. 

 

For All First Year Athletes (this includes transfers) - Access the e-PPE website from any computer that has an internet connection.  each new student athlete will have to go through the "new athlete registration process" There are 5 stages:
1. Registration using an email account and password
2. Complete the demographic information
3. Complete the health history questionnaire
4. ATTACH yourself to the appropriate team - under manage team memberships
5. Print your forms A,C,D, these forms are to be used for your Physical Examination.
 
After the student registers by providing personal profile information, they begin answering the questions that address issues such as current and past medical problems, injuries, family history, medications, allergies and eating habits.  Depending on the responses to key questions more detailed information may be requested. Each section must say 100% complete before the student will be able to print your forms. Once the student completes all sections of the questionnnaire they are instructed to print out all necessary releases and summaries and physical examination forms. 
Forms A, C, D must be printed and taken to the physician performing your physical. The physician and the athlete signs off on Form A. Once completed Form A must be returned to (coach or IA office) 
 

 
For All Returning Athletes - you will be responsible for completing an update form - download from Western Mustangs site. This form must be returned to the coach or the Athletics office prior to your first practice.
