- Certificate of entrustment should state the signatory's personal information, date of birth, ID, place where the items were lost, and description of the items.
- It should also include the signatory's passport number, address on the articles, contents of the articles with condensed descriptions, pattern number, special features, and amount, and must be signed.
- Certificate of entrustment from fax should contain the signatory's chop.
- The receipt must match firmly with the certificate of entrustment, and must be put down with the received material in detail.
- If the major articles of lost (money, camera and etc) are different from the one stated on the certificate of entrustment they will not be returned.
- The trustee must sign the receipt.
PS: If the articles lost are all checked and returned, there should be attached a note and comment for the records.

- Passport or other identification with your photo.
- Personal chop.
- A copy of your passport.
