Methods of Reporting Disease:
Infectious Disease Reporting

The Minnesota Department of Health offers multiple ways of reporting infectious diseases. Any reportable disease can be reported by telephone, some diseases must be reported immediately by phone and MDH uses three forms. In addition, some diseases require additional information that is not requested in the Yellow Card, for those disease there may be a supplemental form requesting additional information.

Phone reporting

  • "24-7" Phone Reporting
    Some diseases must be reported immediately by phone 24 hours a day, 7 days a week.

  • Phone Reporting
    Any reportable disease can be reported by telephone to 651-201-5414.

Reporting forms

  • Yellow Card
    The MDH "Yellow Card" (previously known as the "blue card") is used to report cases of most reportable infectious diseases in Minnesota residents to the Minnesota Department of Health.

  • STD Confidential Case Report Card
    The Minnesota Confidential STD Case Report Card is used to report cases of chancroid, chlamydia, gonorrhea, and syphilis in Minnesota residents to the Minnesota Department of Health.

  • HIV/AIDS Case Report Forms
    The HIV/AIDS Confidential Case Report Form is used to report cases of HIV/AIDS in Minnesota residents to the Minnesota Department of Health.

  • ABCs Case Report Form
    The Active Bacterial Core Surveillance (ABCs) Case Report Form is used to report cases of invasive bacterial diseases in Minnesota residents to the Minnesota Department of Health.

Supplemental forms

For most supplemental reporting forms, a Yellow Card is also required.

Proposed Amendment to the Communicable Disease Reporting Rule
The Minnesota Department of Health is considering an amendment to the Communicable Disease Reporting Rule, Minnesota Rules, Chapter 4605. The change would require all laboratories to report to the Minnesota Department of Health the results of all CD4+ lymphocyte counts and percents and the results of all HIV viral detection laboratory tests.
