There are several ways to make an appointment with the Department of Ophthalmology:

1. Send an email to the Scheduling Center at oph-scheduling@healthcare.uiowa.edu with the information requested below. We will contact you by return e-mail or by phone. (Please keep in mind that e-mail is not secure.)
2. Call the Department of Ophthalmology Scheduling Center at 319-356-2852. Due to heavy volume, you may get a busy signal. Please fill out form to request that we return your call.
3. Write a letter to:

   Scheduling Center

   200 Hawkins Drive

   Iowa City, IA 52242

4. Visit the Ophthalmology Scheduling Center:
   Room 11221
   First Floor
   Pomerantz Family Pavilion
5. Fill out the Appointment Scheduling Request Form.

For an appointment with Pediatric Ophthalmology or Strabismus Service:

1. Call the Ophthalmology Pediatrics Scheduling Center at 319-356-2859.
2. Write a letter to:

3. Visit the Pediatric Ophthalmology Scheduling Center:

The following information is necessary for an appointment with the Department of Ophthalmology:

• Full name with correct spelling
• Social security number
• Date of Birth
• Mailing address
• Daytime phone number
• Chief complaint or reason for appointment
• Names of caregivers (if any) to whom a report should be sent
